Objective To analysis the value of HPV E6/E7 m RNA in the diagnosis of cervical lesions.Methods Totally 197 patients who had the positive results of High-risk HPV,and Thin-prep liquid-based Cytology Test(TCT)is Benign cytological changes in Yijishan Hospital of Wannan Medical College gynecology clinic from September 2014 to March2016 were examined with b-DNA technology to detect HPV E6/E7 m RNA,while each of them underwent the colposcopy and cervical biopsy.Analyze the relationship between the copy value of E6 / E7 m RNA in cervical lesions and pathological outcomes;detect the expression of HPV E6/E7 m RNA in single and multiple infection;to explor the relationship of the expression of E6 / E7 m RNA in high-risk HPV and pathological results.Results 1.Among 197 patients,86 cases were inflammation.Cervical intraepithelial neoplasia Ⅰ(cervical intraepithelial,neoplasia,CIN)in 48 cases,CINⅡfor 38 cases,and 25 cases for CIN Ⅲ.142 cases were found HPV E6 / E7 m RNA positive,the positive rate of HPV E6 / E7 m RNA was 72.08%,the sensitivity of HPV E6 / E7 m RNA was 90.99%,the specificity was 52.32%,the positive predictive value was71.13% and the negative predictive value was 81.82%.The positive rate of HPV E6/E7 m RNA in inflammatory group and CIN Ⅰ,inflammatory group and CIN Ⅱ,inflammatory group and CIN Ⅲ were both statistically significant.With the increase of the degree of cervical lesions,the copy of the E6/E7 m RNA showed a rising trend.In high grade lesions(≥CIN Ⅱ),the copy value of E6/E7 m RNA was significant high than low grade lesions.2.Among the 197 HR-HPV positive women,there were 111 cases of single type infection(56.35%),and 86 cases were infected with multiple types(43.65%).Totally 101 cases were found HPV E6/E7 m RNA positive,the positive rate was 71.13%,while only 28.87% in inflammation cases.The expression of E6/E7 m RNA of multiple type infections was higher than single type infections between inflammatory group and CIN Ⅰ,inflammatory group and CIN Ⅱ,inflammatory group and CIN Ⅲ,and result were both statistically significant.3 Among all cases,HPV16 was the most predominant type.There were 81 cases was HPV16positive(25.16%),which include 22 cases of single type infection;and 72 cases of HPV52 positive(22.36%),21 of which were single type infection;and 16 cases were single type infection of 64 cases of HPV18 positive(19.88%).We found there was no significant different of the expression of E6 / E7 m RNA between human papillomavirus genotype.Among human papillomavirus genotype 16,52,18 group,the expression of E6 / E7 m RNA of inflammatory group was significant higher than cervical intraepithelial group.Conclusions 1.The positive expression rate of E6/E7 m RNA in different grades of cervical lesions was significant different,and the copies tend to be increased with the degree of cervical lesions.The transcripion of HPV E6/E7 m RNA was closely related to the level of cervical lesions.The detection of E6/E7 m RNA was helpful to evaluate the degree of cervical disease and malignant development trend.2 There are differences of the expression of E6/E7 m RNA between multiple infection and single infection in different grades of cervical lesions,presence of m RNA was increasing in multiple infection.3.HPV16,52,18 were the most common human papillomavirus genotypes in this study,and the high transcription of E6/E7 m RNA was found in these common high-risk types.Also the expression of E6/E7 m RNA was significant higher in cervical intraepithelial group,especially with multiple types infection.HPV E6/E7 m RNA is closely correlated with cervical lesions,it can play a key role in screening precancerous lesion and early cervical cancer. |
