| Objective This article is to investigate the age distribution and infection characteristics of different types of high-risk HPV infection in women in Hefei area,and to study the significance of HPV E6/E7 m RNA detection in cervical cancer screening.Methods The types of HPV infection in women screened for cervical cancer in Anhui Maternal and Child Health Hospital from January 2020 to December 2020 were statistically analyzed.Some of the patients with high-risk HPV positive results and TCT results showing cervicitis or above were performed colposcopic cervical biopsy and recorded the results.Meanwhile,some of the patients who underwent colposcopic cervical biopsy were performed HPV E6/E7 m RNA detection and the results were counted.Results 1.Among the 15,931 women who underwent cervical cancer screening,2907 of them tested positive for high-risk HPV(18.25%).Among them,the high-risk HPV group over 56 years old had the highest infection rate and multiple infection rate(25.71% / 30.09%),followed by women under 25 years old(21.71% / 27.34%).The infection rate of high-risk HPV group in both groups was significantly higher than that in 26-55 year old women(P < 0.016).Females under 25 years of age showed the highest positive rate of HPV16/18(30.8%).The positive rate of HPV16/18 decreased with age.THRee HPV subtypes showed the highest population infection rate and single infection rate: HPV52(4.24% / 62.67%),HPV16(3.43% / 63.55%),and HPV58(3.23% /61.28%).Among the lesions above CIN II,the positive rates of t HRee KINDS of HPV:HPV52,HPV58 and HPV53 were relatively low,and the positive rate of HPV52 decreased gradually when the cervical lesions progressed from CIN I to cervical cancer.2.A total of 360 patients underwent HPV E6/E7 m RNA detection,of which 195 were positive for HPV E6/E7 m RNA detection,the total positive rate was 54.17%,including cervicitis group,CIN I group,CIN II III group and cervical cancer group.The positive rates were 42.04%,54.55%,70.10% and 75%,respectively.The positive rate of CIN II III group was significantly higher than that of cervicitis group and CIN I group,and the difference was statistically significant(P < 0.016).The TCT results were atypical squamous cells of undetermined significance(ASCUS)and Atypical Squamous Cells-HSIL cannot be excluded(ASC-H),The positive rate of CIN II III was significantly higher than CIN I and cervicitis groups,and the difference was statistically significant(P < 0.016).The area under ROC curve of HPV E6/E7 m RNA detection was 0.644,and the optimal operating point was 871.105copies/ m L of HPV E6/E7 m RNA,at which time the sensitivity of HPV E6/E7 m RNA detection of high-grade lesions was 62.6%.The specificity was 65.3%.The copy number of HPV E6/E7 m RNA increased gradually with the grade of cervical lesions,and the CIN group was significantly higher than the cervicitis group,with statistical significance(P <0.05).Conclusions 1.The infection rate of high-risk HPV in women in Hefei is at a low level in China,presenting a "bimodal distribution" of high infection rate in young women and elderly women.HPV52,HPV16 and HPV58 HPV subtypes have the highest infection rate and single infection rate in population.The high proportion of single infection may be one of the reasons why these t HRee types of HPV and HPV18 tend to form persistent infection and account for a high proportion of patients with HSIL.2.HPV E6/E7 m RNA detection has a high negative predictive value for HSIL detection in cervical cancer screening,which can be used for shunting patients with TCT results of ASC-US and ASC-H. |
