Effects Of Propofol TCI Induction On Hemodynamic In Elderly Patients

Objective: Comparison of different plasma target controlled concentrations of propofol combined with fentanyl in Narcotrend monitoring.To explore the effect of general anesthesia induction on hemodynamics in elderly patients,and to explore the plasma target controlled concentration of propofol combined with fentanyl in general anesthesia induction period.Methods: Under general anesthesia,aged 65~80 years old elderly patients undergoing elective surgery in 60 cases,ASA grade I ~ II,according to the induction period of the plasma concentration of propofol were randomly divided into three groups,20 cases in each group;group S1,propofol target plasma concentration of 1.5 ?g/ml;group S2,propofol target plasma concentration of 2.0?g/ml;S3 group that propofol target plasma concentration of 2.5?g/ml.induced anesthesia midazolam fentanyl propofol and rocuronium combined intravenous anesthesia method.With TCI pump by target controlled infusion of propofol,the propofol target plasma concentration reaches a preset target concentration,the patients lost consciousness after intravenous injection of rocuronium 0.8mg/kg when the Narcotrend value reached 50,and the following tracheal intubation control breath with muscle;the induction process,if SBP< = 30%,intravenous injection of ephedrine 5mg,HR<55 / min,atropine 0.25 mg,record the basic value(T0),before intubation(T1),immediately after intubation(T2).The trachea After intubation1m(T3),after intubation 3m in(T4),systolic blood pressure(SBP),diastolic blood pressure(DBP),mean arterial pressure(map),heart rate(HR)and anesthesia induced by atropine and ephedrine in the course of the frequency of use.Results:Hemodynamic effects: compared with t0.Three groups at time T1 SBP,DBP,map,HR were decreased(P < 0.05);compared with T0,T1 and T2 in group S2,T3,T4 moments of SBP,DBP,map,HR changes no statistical significance(P > 0.05),compared with group S2,S1 group at T2,T3 SBP,DBP,map,HR time increased significantly(<P0.05).S3 group at T2,T3,T4 moments of SBP,DBP,map,h significantly decreased(P < 0.05).S3 group of ephedrine and atropine used times higher than the other two groups(P < 0.05).Conclusion: Propofol combined with fentanyl target controlled infusion in elderly patients undergoing general anesthesia induction is safe and suitable for the blood Plasma target concentration is 2 g/ml.
