| Objective: Using virtual touch tissue quantification(VTQ)to detect the changes of renal cortex SWV in patients with chronic glomerulonephritis,And according to the panel of e GFR recommended clinical stage,to evaluate the technology could provide some clinical value for the early diagnosis and the degree of renal damage in patients with chronic glomerulonephritis.Method: Using two-dimensional ultrasound and color Doppler ultrasound to make Routine examination for 58 cases of early chronic glomerulonephritis patients on the right side of the kidney,The size,shape,boundary,internal echo and blood flow distribution of the kidney were observed and detected.Getting the largest section of the long axis of the right kidney,to open VTQ technology in ARFI mode and measure the velocity of transverse shear wave in the right renal cortex(that is,the SWV value),repeated it 3 times to find the average value of the sample(The operation of the process should ensure that the area of interest(ROI)and the distance of the probe is not more than 5.5cm).To storage image and record the measured values,a retrospective analysis was made on the pathological results.Drawing ROC curve of the AWV value in II phase of chronic glomerulonephritis accord to the clinical stage recommended by the e GFR task group,to judge the clinical diagnostic value of the index by the area under the curve and find the best value point.Result:(1).The outcome which according to the result of pathological grouping analysis:comparison of the mean value of SWV between the chronic glomerulonephritis group(includ group A:primary glomerulonephritis;group B:lupus nephritis)and the control group:The mean value of SWV(SWVIn)in group A was(3.13±0.85)m/s;The mean value of SWV in group B(SWVIn)was(3.07±0.97)m/s;The mean value of SWV in the control group(SWVIn)was(3.82±0.15)m/s.Compared with the control group,the group A and group B had statistical significance(P<0.05),but there was no statistical significance in the group of A and B(P>0.05).(2).The outcome which according to the grouping GFR values analysis: comparison of the mean value of SWV between early chronic glomerulonephritis group(includ group C:the I phase CGN I;group D:the II phase CGN II)and control group:The mean value of SWV(SWVIn)in the group C was(3.47±0.69)m/s;The mean value of SWV(SWVIn)in the group D was(2.69±0.93)m/s;The mean value of SWV in the control group(SWVIn)was(3.82 ± 0.15)m/s;the group D cortex SWV was significantly lower than that of the group C and the control group,and the difference has statistical significance(P<0.05);Comparing the group C and control group,there was no significant difference in the SWV of renal cortex(P=009).(3).Accoding to the ROC curve analysis of this study :SWA 3.45m/s is the optimal critical value of CGND in the diagnosis of II,The corresponding sensitivity and specificity were:83.9%、66.7%,The area under the ROC curve is 0.732.Conclusions: 1.For the Chronic glomerulonephritis patients,with the renal damage,renal elasticity and compliance decreased,and it performanced that the SWV value of renal cortex decreased.2.It can reflect the degree of renal damage in early CGN to a certain degree of quantification by using VTQ technology to detect SWV value,which provide a new reference for the early diagnosis of CGN.3.In this group,according to the panel of e GFR recommended clinical stage,drawing the ROC curve to use SWV value to evaluate CGN II,and obtained 3.45m/s is the best critical value of early diagnosis of CGN. |
