Clinical Study On Treatment Of Acromi Oclavic Ular Joint Dislocation By Double Endobutton Plate Fixation Under The Whole Arthroscope

Objectives: To study surgical treatments of acromioclavicular joint dislo cation.We compared the efficacy of treatment by traditional clavicle ho ok plate fixation with that by double Endobutton plate fixation under th e whole arthroscope,therefore the optimal therapy would be selected to provide a preferable treatment scheme for acromioclavicular joint disloca tion.Method: In this study,we selected a total of 60 patients with acromio clavicular joint dislocation at Rockwood Type III and above,and used th e table of random number to divide them into two groups.They were: treatment group which consists of 30 patients,including 15 men and 15 women,treated with double Endobutton plate fixation under the whol e arthroscope,and control group which consists of 11 men and 19 wom en treated with traditional clavicle hook plate fixation.We conducted st atistics analyses to patients in both groups on the wound healing and s houlder braking times after surgery,CMS shoulder functional scales and complications of shoulder joint at the third month after the surgery,an d x ray films for shoulder joint reexamination at the third month after the surgery.We applied statistics software SPSS19.0 to carry out statisti cal analysis of the clinical efficacy and make final judgment on the eff icacy.Results: During this study,in terms of the wound healing after surgery in the two groups,30 patients in the treatment group all had grade A healing while there were 4 patients had grade B healing in the control group.This indicated that the treatment group had a better healing tha n the control group.With regard to the braking time after surgery,it w as an average of 15.10±1.74 days in the treatment group and 16.80±2.12 days in the control group;P?0.05,therefore the differences between t he two groups were statistical significance and the shoulder braking tim e after surgery in the treatment group was shorter than that in the cont rol group.In comparing the CMS shoulder functional scales and compli cations of shoulder joint at the third month after the surgery,the treatm ent group had better results in pain,daily activities and sphere of activi ties than the control group.P<0.05 means that differences were statistic al significance in those three aspects mentioned above;in the third mon th after the surgery,there were 5 cases of impingement syndrome in th e control group while the treatment group had none.When observing t he x ray films of shoulder joint at the third month after the surgery,th ere was 1 case of breakage,1 unhooking and 5 cases of wear of suba cromial bone in the control group;there was neither case of migration and loosening of plate nor wear of subacromial bone in the treatment g roup.Conclusion: The results of this subject study show that the treatment of acromioclavicular joint dislocation at Rockwood Type III and above u sing double Endobutton plate fixation under the whole arthroscope have such advantages as better healing,fewer complications,less braking ti me,and better recovery of shoulder function after surgery;it provides a new effective clinical treatment for acromioclavicular joint dislocation.
