Backgroud and ObjectiveThe proposed pathogenesis of COPD includes immunological mechanisms、 systemic inflammation, chronic airway inflammation and progressive development of airflow obstruction are two important features of COPD. Domestic study found that interleukin-6(IL-6) in peripheral blood with COPD at stable stage increased notablely, to a certain extent, it also reflects the degree of systemic inflammation in patients with COPD, and the negative correlation with FEV1, it could be an effective indicator to predict COPD patient’s condition and prognosis. C- reactive protein(CRP) is another cytokine that related with COPD closely, and synthesis and secretion by the liver cells, when the body in inflammation response, injury and surgery, the CRP levels increased rapidly. Adiponectin(APN) is a cells cytokines that mainly secreted by fat cells, is related closely with increased tumor necrosis factor(TNF-α) and IL-6.The pathogenesis of COPD is still unclear, however, the theory of chronic inflammation is thought to be the one of the main mechanisms of the COPD. The phenomenon of chronic inflammation exsited in airway、lung parenchyma and pulmonary vascular of COPD patient, especially in the acute exacerbation stage, airway inflammation in a high state response, neutrophils, macrophages and lymphocytes activated inflammation cells and the secretion of inflammatory mediators, cytokines involved in the pathogenesis of COPD, but the specific mechanism of various inflammatory factors and its correlation with lung function is not yet clear. COPD often leads to serious complications, and even threaten their life. Therefore, it is of great significance to determine the severity and prognosis of patients at early stage.To investigate the changes of serum adiponectin(APN)、C-reactive(CRP)、interleukin-6(IL-6) and body mass indexin(BMI) male patients chronic obstructive pulmonary disease( COPD) at stable stage, and explore their relationship with BMI and pulmonary function(PF), in order to provide new evidence for the pathogenesis、prevention and diagnosis of COPD. Methods108 male COPD patients at stable stage were chosen as COPD group, and at the same time 60 health male volunteers in heath collected in medical center of zheng zhou University second hospital as control group. Each character of all subjects including age, length, weight were also recoded, and BMI was calculated, and were distinguished to mild, moderate, severe and very severe four subgroups by limited airflow in degress after the Bronchial relaxation experiments.Enzyme-linked immunosorbent measurement(ELISA) was used to detect levels of serum APN and interleukin-6(IL-6).Immune turbidimetric method is used to detect all the reseach object of C-reactive protein(CRP).And a spirometer named Pulmonary Function Equipment(COSMED) was used to obtain all lung fuction measurements including FEV1% predicted value, FVC %predicted value, FEV1/FVC(%)were automatic generated by computer system.Relevant statistical testing was used to find the difference of serum APN、IL-6、CRP in two groups.Correlation methods of statistical analysis were used to study correlation between serum APN and IL-6、CRP, the correlation of BMI and pulmonary function. Results1、The levels of serum IL-6、CRP、APN in COPD male patients increased significantly as compared with control group(P<0.01), the fomer BMI,FEV1%pred,FVC% pred,FEV1 / FVC(%)than the control group significantly decreased,and the difference was statistically significance(P<0.01).2、According to classification of lung function groups of COPD male patients serum IL-6,CRP,APN levels are higher than the control group,in addition to mild COPD subgroup,the difference had statistical significance(P<0.05),the worse lung function classification,the lower BMI,IL-6,CRP and the higher the level of APN(P<0.05).3、APN were negatively correlation with BMI and lung function(P<0.05),and positive correlation with IL-6 and CRP(P<0.05);BMI was negatively correlation with APN(P<0.05),and IL-6,CRP and lung function indexes are related(P<0.05) Conclusion1、The levels of serum APN in stable stage of COPD patients increased, and correlated significantly with inflammatory factors. APN may be participated in the occurence and development of the systemic inflammation in patients with COPD, and correlated significantly with FEV1%pred、FVC%pred、FEV1/FVC% and BMI.2、The serum APN in patients with COPD at stable stage related closely with BMI, APN likely to be involved in patients with COPD of deterioration, malnutrition. The level of serum APN can be used as an index to evaluate the degree of severe COPD.
