Tai Chi And Whole-Body Vibration Therapy On Elderly:A Clinical Randomized Controlled Trial

Objective To observe the effect of Tai Chi and Whole Body Vibrating Training on muscle mass、muscle strength and physical function in the aged, and discuss the superiority between different therapeutic methods.Methods Part Ⅰ 60 subjects (25 male,35 female) aged 64 years old were recruited and assigned to the Tai Chi(TC) group and Control group(imparting health knowledge) by random digit table. People in the TC group received 24-style Tai Chi Chuan training for 18 months(60min each time,5 times per week), while those in the control group were imparted with sarcopenia related knowledge. The maximum isometric strength of iliopsoas, quadriceps femoris, tibialis anterior muscle, hamstrings; the time for walking speed, TUGT, FTSST, one-leg standing time with closed eyes test; and the score of Berg balance scale were compared between the two groups. Part II 90 hospitalized patients with sarcopenia (age≥85 years) were recruited and randomly assigned to the Tai Chi(TC) group, whole body vibration therapy (WBV) group, and control(CON) group by Zelen method. People in the TC group and WBV group received Tai Chi Chuan and whole body vibration therapy for 8 weeks respectively, while those all of patients in three groups were imparted with sarcopenia related knowledge. The muscle mass, muscle strength, walking ability, balance ability, bone mineral density, barthel index, anxiety, depression condition and cognitive ability were compared among these three groups.Results:Part I Compared with before treatment, muscle strength increased in the TC group as follows, iliopsoas 5.5Kg (16.9%), quadriceps femoris 5.5Kg (26.2%), and tibialis anterior muscle strength 8.5Kg(36.2%)(P<0.05). The time for walking speed was increased by 45.2%. The time for TUGT and FTSST was shortend by 16.7% and 14.5% respectively in the TC group. The time for one-leg standing time with closed eyes test was increased by 8.4s(left) and 9.1s(right) respectively. The score of Berg balance sacle increased by 4.3% (P<0.05)。Compared with the control group, the observed indicators were all improved in the TC group, except hamstring strength.Part Ⅱ Compared with before treatment, the increase of muscle mass, bone mineral density is insignificant. The handgrip strength increased in WBV(12.8%) group, while there is no change in TC and CON groups. The strength of quadriceps femoris (15.0% & 15.6%) and tibialis anterior muscle (14.3% & 18.2%) improved significantly in both TC group and WBV group, while iliopsoas’strength (17.2% & 5.6%) improved significantly in TC group, and hamstrings’strength (16.3% & 6.7%) improved significantly in WBV group. The enhancement of static (150.4% & 123%) and dynamic (54.7% & 45%) balance ability in TC group was better than that of WBV group after 8-week therapy. The walking speed, TUGT, FTSST, the number of frail people, HAMA and HAMD improved in TC group and CON group (P<0.05), except the cognitive ability.Conclusion1. Tai Jiquan exercise can prevent the lower limb muscle strength from declining, and it can increase walking speed, shorten the time for TUGT and FTSST test and improve the balance ability in elderly. It suggests that Tai Jiquan exercise will contributes to the prevention of sarcopenia.2. There is no significant improvement in muscle mass and bone mineral density during 8-week intervention.3. The handgrip strength increased in WBV group, while there is no change in TC and CON groups. The strength of quadriceps femoris and tibialis anterior muscle improved significantly in both TC group and WBV group, while iliopsoas’strength improved significantly in TC group, and hamstrings’strength improved significantly in WBV group.4. The walking ability, balance ability and frail, anxiety, depression condition and Brethel index of aged sarcopenic men improved significantly in TC group and WBV group. The enhancement of balance ability in TC group was better than that of WBV group after 8-week therapy.It indicates that we can choose the proper thrapy according to the targeted muscles. cognitive degree and the physical condition.
