Background: Bone metastasis cancer is tumors which primitively generated from other regions through different methods.The clinical manifestation of bone metastasis cancer is pain,pathological fracture,hypercalcemia and so on.CIBP(cancer-induced bone pain)has wide influence on the body system,to relieve pain effectively can significantly improve the life quality of cancer patients,thus it is one of the main purpose of the clinical treatment of bone metastatic cancer.The pathological mechanism of CIBP is complex,there are studies found that it’s related with the peripheral and central sensitization,not only has the characteristics of neuropathic pain,but also has inflammatory pain characteristics,traditional analgesia methods cannot make CIBP fully under control.The current clinical analgesic method to control pain is drug combinations,including non-opioid drugs,opioid and external irradiation,internal irradiation of radiotherapy,bisphosphonate therapy and palliative surgery.But current methods to control pain could not solve clinical question.Hyperthermia is a physical therapy,has be used in clinical application widely.In recent years,technology and device have a great development,for deep hyperthermia,people gradually has a further recognition.Deep hyperthermia can penetrate deeper tissues,for deeper tumors,can increase the effect of treatment.Current reports related with deep hyperthermia cover varied kinds of tumors,but few reports related to bone cancer,so we need to have further research for this place.Objective: To observe the effect of BSD 2000 phased array deep hyperthermia in the improvement of pain and quality of life for patients with CIBP.Make a preliminary discussion about the mechanism of CIBP and how the hyperthermia improving bone pain and quality of life with or without other treatments.Method: Make a collection of 7 patients who use BSD 2000 phased array hyperthermia treating the bone cancer pain in the 2015.2-2016.1of the second hospital of Dalian Medical University,all patients were under anti-tumor treatment,5 patients with oral analgesics for pain relief,the effect is poor,all patients have no contraindication and sign the consent.All the patients have used the BSD 2000 phased array deep hyperthermia system,according to the CT、MR of patients,we fixed the position of the tumor,and find the surface projection area of the tumor,then arrange in the measuring needle on it,input the tumor coordinate and other numbers into the computer system.Before and after treatment we will make pain score according to the digital method(numerical rating scales,NRS)for painful patients,each patient for at least 1 cycles(10times)hyperthermia,to observe the improvement of quality of life according to the scores of quality of life of patients who have tumors,according to the imaging examinations and RECIST criteria to evaluate the short-term efficacy of the solid tumor and to observe the adverse reactions during the treatment.Result: 7 patients have finished a total of 220 times therapy,each one completed at least10 times,namely 1 courses,including 1 cases of breast cancer patients with bone cancer pain completed a total of 120 times,60 times the left and right femur,all the 7 patients have relieved the pain after treatment,before treatment 5 patients have outbreak pain about 2-3times one day.Combined with BSD 2000 phased array hyperthermia after 1course(10 hyperthermia),compared with before,the frequency reduced to 0-1 times,after 2 courses of therapy(20 hyperthermia),3 patients have relieved the pain seriously,stopped taking analgesic.2 breast cancer patients with a slight pain,which can be tolerated,complete remission.All the 7 patients can evaluate the short-term efficacy and adverse reactions,3 cases are bone metastasis of breast cancer,all patients had no adverse reactions of the original treatment.Conclusion:1.BSD 2000 phased array deep hyperthermia is an effective method for cancer-induced bone pain,in combine with other methods can increase the quality of life.2.BSD 2000 phased array deep hyperthermia is an accurate conformal hyperthermia which not increase the side effect of other method.As treatment starting,patients will sweet slightly,it’s worth of research and spread.3.BSD 2000 phased array deep hyperthermia relieving cancer-induced bone pain is related to the mechanism of killing cells,promoting cell apoptosis,accelerating metabolism,reducing the nerve sensitiveness,it need further research. |
