| Objective: To study before and after nasal polyps use fluticasone propionate nasal spray, Then observe the differences of nasal polyps eosinophil infiltration in nasal polyp tissues and make comparison with those in normal turbinate tissue, as well as the expression of IL-5(interleukin-5) and GM-CSF(granulocyte-macrophage colony stimulating factor), and to understand the degree of eosinophilic infiltration during the formation of nasal polyp, the function of expression of IL-5and GM-CSF and the influence of nasal hormone on nasal polyp tissues, further to reveal the mechanism of glucocorticoid treating nasal polyps and its significance in clinical application. Methods: to detect the expression of IL-5 and GM-CSF in nasal polyps and normal turbinate tissue before and after using fluticasone propionate nasal spray by immunohistochemistry and computer image analysis; to test the degree of eosinophil infiltration in nasal polyps and normal turbinate tissue before and after using the auxiliary propionate nasal spray via hematoxylin-eosin staining; to make correlation analysis between IL-5, GM-CSF measured in each nasal polyps(including before and after the application for Auxiliary propionate nasal spray) and the degrees of eosinophil infiltration by drawing scatterplot. Results: 1. The expression of IL-5, GM-CSF and the degree of eosinophil infiltration were increased in nasal polyps compared with normal turbinate tissue, P <0.01 difference was statistically significant; 2. Making comparison before and after using fluticasone propionate nasal spray, the expression of IL-5 and GM-CSF and the degree of eosinophil infiltration was significantly reduced, to make pairwise comparison, P<0.01 difference was statistically significant; 3. the expression of IL-5 and GM-CSF in nasal polyps and eosinophil infiltration level were positively correlated(before and after using the fluticasone propionate nasal spray, the correlation coefficient between the expression of IL-5 and the degree of eosinophil infiltration respectively werer=0.79, p <0.01 and r = 0.877, p <0.01; before and after using the fluticasone propionate nasal spray, the correlation coefficient between the expression of GM-CSF and the degree of eosinophil infiltration respectively were r = 0.723, p < 0.01 and r =0.959, p <0.01). Conclusions: 1. The expression of IL-5 and GM-CSF in nasal polyps were increasesd, and its eosinophils infiltration level was also increased, which suggesting that the expression of IL-5 and GM-CSF may increase the eosinophils infiltration level and play a role in the formation of nasal polyps; 2. After using fluticasone propionate nasal spray, the expression levels of IL-5 and GM-CSF in nasal spray were significantly reduced indicating that fluticasone propionate nasal spray can effectively control the expression of IL-5 and GM-CSF in nasal polyps, thereby reducing the eosinophil infiltration and further inhibiting the development of nasal polyps and promoting cell apoptosis in nasal polyps tissues; 3. further confirmed that the application of fluticasone propionate nasal spray before surgery can reduce the difficulty of operation, and prevent blood loss. |
