Objective:To discuss the relation of 64-slice spiral computed tomography angiography of anterior communicating aneurysms and the risk of rupture, and try to predict the risk of the rupture of anterior communicating aneurysms.Methods: The materials about the 64-slice spiral computed tomography angiography of Sixty-eight patients who have anterior communicating aneurysms from September 2013 to December 2015 in Changsha Central Hospital are collected.All the patients are examined by using intracranial CTA within 24 hours after the onset of the disease.All the data are reconstructed based on three-dimensional technology with the layer thick of 0.625 mm and layer distance of 0.625 mm, then processed by processing software.At first, with the reconstruction image of GEAW4.5 3-D reconstruction workstations,record the asci the locations of anterior communicating aneurysms; then,measure the length, the width and the width of the neck,calculate the ratio of the length with the width of the neck(AR)and the ratio of the width of the neck, calculate the ratio of the length with the width of the neck(AR) and the ratio of the width with the width of the neck(BN); at last,analyze the data by using SPSS17.0.Results: The 68 patients involved in the experiment have 73 anterior communicating aneurysms, among which there are 47 ruptured ones, 26 unruptured ones, 20 saccular aneurysms and 16 shuttled aneurysms.1. The locations of anterior communicating aneurysms: there are 24 on the left, 19 of which are ruptured, the rate of rupture is 79.17 percent; there are30 in the middle, 12 of which are ruptured, the rate of rupture is 40 percent;there are 19 on the right, 16 of which are ruptured, the rate of rupture is 84.21 percent. It’s obvious that the rate of rupture on the right is the highest; the differences among the three rates are statistically significant(Χ2 =173.01,P< 0.05). The rate of rupture(81.4 percent) on the sides(left and right) is obviously higher than the rate of rupture in the middle(40.00 percent); the differences between the two rates are statistically significant(Χ2 =13.21,P<0.05).2. The length of anterior communicating aneurysms(L): L≤3mm, 3<L≤7, L>7mm. The rates of rupture about these three groups are 27.27 percent,69.57 percent and 75.00 percent. The differences between the three rates are statistically significant(Χ2 =7.71,P<0.05). The rates of rupture on the first two groups and the third group are 61.40 percent and 75.00 percent; thedifferences between the two rates aren’t statistically significant(Χ2=0.0086,P>0.05).3. The analysis of the ruptured ones and the unrupured ones: The length of anterior communicating aneurysms(L)(5.95±2.77 mm vs 4.40±2.22mm),the ratio of the length with the width of the neck(AR)(2.22±0.93 vs 1.65±0.49) and the ratio of the width with the width of the neck(BN)(1.66±0.61 vs 1.37±0.55) are statistically significant(P<0.05), the width(W)(4.50±1.90 mm vs 3.69 ± 2.13mm) and the width of the neck(Wneck)(2.86 ±1.10 mm vs 2.77±1.51mm) aren’t statistically significant(P>0.05).4.The existence of ascus: there are asci existed in 30 anteriorcommunicatinganeurysms, and 26 of which are ruptured, the rate of rupture is 86.67 percent. There are 43 anterior communicating aneurysms without ascus, and 22 of which are ruptured, the rate of rupture is 48.84 percent. The differences between the two rates are statistically significant(Χ2=11.02,P<0.05).Conclusion: 1. The risk of rupture on anterior communicating aneurysms at the crotch is higher than the rate of the ones not on the crotch.2. The length, AR and BN of anterior communicating aneurysms is influence factors of aneurysm rupture. The bigger the ratios are, the higher the risk of rupture is. The formation of ascus is also one of the factors of anterior communicating aneurysms rupture.
