Objective:This research compares the difference of success rates of implant, the amount of the implant neck bone resorption, ISQ, probe depth, satisfaction of patients in different periods(time points) of local bone loss caused by chronic inflammation and dental caries(accident) group. This research further explored the feasibility of immediate implant of patients with chronic inflammation around the teeth.Methods:66 patients participated in our study. They were divided into two groups:chronic inflammation around the teeth group of 30 patients, dental caries(accident) group of 36 patients. After local anesthesia, extract the tooth root with minimally invasive, clean the inflammatory tissue during surgery, then the patients take oral antibiotics for 5 days after the surgery, implant in the same period. Implant permanent fix was done 6 month after surgery, we measured the success rates of implant, the amount of the implant neck bone resorption, ISQ, probe depth, satisfaction of patients at 3, 6, 12, 18 months after the surgery.Results:The mean age of the research group is 55.80±11.22, sex rate 3:2; the mean age of the control group is 53.66±14.18, sex rate 5:4. There was no significant difference of age and sex in two groups.1. The success rate of immediate implant is 100% in both groups,with goodosseointegration and function.2.The bone resorption amount of research group and control group was respectively at 3 month and 6 month were: 0.38±0.05 mm and 0.37±0.04mm(P =0.267);0.21±0.03 mm and 0.20±0.03mm(P =0.415), 12 months and 18 months were: 0.51±0.03 mm and0.49±0.04mm(P =0.070);0.10±0.02 mm and 0.09±0.02mm(P =0.071)3.The ISQ of the research group did not differ from the control group at 6 months,70.83±6.62 and 71.56±5.96(P=0.643).4.The probe depth of two groups at 6 month, 12 month, 18 month was1.80±0.07 mm and 1.77±0.06mm(P =0.054);2.01±0.05 mm and 1.98±0.09mm(P=0.205);2.18±0.20 mm and 2.17±0.19mm(P =0.778).5.There was no significant difference on satisfaction of research group and control group except aesthetics(P=0.041).Conclusion:After 18 months’ research, it is found that the immediate implant of chronic inflammation around the teeth is feasible, with good osseointegration. And research of its long-term effect is needed. Part of the patient’s teeth of soft tissues around poor aesthetic effect due to chronic inflammation, suggested that preoperative or operative increase in soft tissue.
