| Objectives To analys the major influencing factors of acute stress disorder(Acute Stress Disorder, ASD) in patients with traumatic fracture and providing evidence for early intervention measures to prevent the occurrence and development of ASD.Methods 440 cases of traumatic fracture hospitalized patients from Tangshan Second Hospital were selected in this study. All subjects were assessed ASD with SASRQ, 110 cases of traumatic fractures were diagnosed as a case group and 330 patients with ASD which did not occur as a control group study. Using the general information questionnaire, the Chinese version of Conner-Davidson resilience scale(CD-RISC), Hospital anxiety and depression scales(HAD), Social Support Rating Scale(SSRS), pain visual analogue scale(VAS), injury severity score(AIS-ISS) to investigate the main factors of ASD in patients with traumatic fracture at the meanwhile. Using Excel software to establish a database and SPSS17.0 statistical software for analysis after checking. Descriptive statistics were used for general information. The chi-square test were used for univariate analysis of general situation, the disease characteristics, anxiety, depression, social support and resilience, etc. Logistic regression analysis were used for multivariate analysis.Results Univariate analysis showed that gender, age, marital status, family income, personality, complications, trauma history, injury severity, degree of fear when injuried, the prognosis expected, illness annoyance level, the worst symptoms days, anxiety, depression, social support, resilience level, the degree of pain related to the occurrence of ASD in trauma fractures patients,the difference was statistically significant(P<0.05).Multivariate Logistic regression analysis showed that gender(OR=2.637), complications(OR=5.872), the degree of fear when injuried(OR=0.629), illness annoyance level(OR=1.545), social support level(OR=2.672) and the pain(OR=4.543)are predictive variables of ASD(P <0.05). The complication is a high risk factor of ASD; Female, high level disease annoyance, low level of social support, strong feelings of pain are risk factors of ASD; Without fear when injuried is a protective factor of ASD.Conclusions 1 Factors of gender, complications, degree of fear when injuried, illness annoyance level, social support and degree of pain are related to occurrence of ASD in traumatic fracture patients. 2 Without fear when injuried is a protective factor of ASD. Female,with complications, severe illness annoyance, low level of social support, strong feelings of pain are risk factors of ASD. |
