Demographic Characteristics Of Idiopathic Membranous Nephropathy

Objective To explore the causes of idiopathic membranous nephropathy in Quanzhou, and then find a solution for the prevention of this disease.Methods Through a self-made epidemiological survey,telephone follow-up and access to medical records, in January 2010 to December 2014 in our hospital who underwent percutaneous renal biopsy for the diagnosis of membranous nephropathy, 79 cases of idiopathic membranous nephropathy were investigated, and access to the relevant information and assistance during the hospital.Results(1) In 79 patients, 45 were male(57%) and 34(43%) were female. 47 patients(59.5%) were over 40 years, 32 patients(40.5%)were aged 40 and younger.(2) The attacking areas were distributed in Nanan, Jinjiang and Quangang district, and the other were distributed.(3) Seasonal incidence in 3-9 months.(3-9 month was the highest Seasonal incidence of the disease.)(4)The incidence of occupation to long-term exposure to dust, pesticides, toxic substances of the occupation, the students also occupy a certain proportion.(5)In 79 patients, smoking, drinking,,family history, history of hypertension of patients with small proportion, a denture implantation, life and work environment were near factory with relatively with large proportion, accounted for 38% and 34.2%.(6)In the clinical manifestation, the majority of 79 patients was accompanied by edema, accounted for 87.3%, renal dysfunction accounted for 32.9%.(7) In laboratory examination, urine routine examination showed simple non nephrotic proteinuria, accounted for 51.9%. 40.5% of the patients with nephrotic syndrome, a few patients showed microscopic hematuria, proteinuria, accounted for only 7.6%.(8)Pathologically, I phase and II phase with membranous nephropathy, accounted for 44.3% and 53.2%, III stage of membranous nephropathy accounted for only 2.5%. With the development of pathological types, of which the prevalence rate of hypertension was 5.7%, 19.4% and 50%, the prevalence rates of renal dysfunctionwere 17.1%, 42.9% and 100%.Conclusions(1) Under 40 years of age the incidence rate of IMN compared with the previous reports in the literature has increased, the proportion of male and female was 1.32:1 which is similar with previously reported, the peak seasons for each year from March to September.(2) Occupation which exposure to dust and toxic and harmful substances and the College Students is the main incidence of this disease. Environmental pollution and toxic exposures may be the main pathogenic factors of the disease.(3) The onset of the disease when multi edema were the main clinical manifestations, routine urine to see non nephritic albuminuria, pathology to phase I and II membrane nephropathy accounted for the majority, and with the progression of Pathology, hypertension and renal function not incidence rate increased significantly.
