| Objective: With traditional classic adjacent finger, abdomen, island flap and in situ replantation surgery methods comparison, research and analysis of clinical therapeutic effect of artificial dermis(PELNAC) through fingertip defect repairMethods: Summarized and analyzed the treatment cases of finger tip defect from June 2013 to December 2014 in hand surgery, as far as possible eliminate differences such as age, nutritional status, economic factors, professional. randomly chosen the clinical data of 20 patients who use the method of Pelnac to repair and cure fingertip defect.In patients who adopt the conventional methods such as adjacent flap, island flap, abdomen flap and in situ implant to cure fingertip defect,randomly selected 20 cases of adjacent flap, 20 patients with abdominal flap, 20 patients with island flap and 20 patients with in situ implant(2 cases were followed up for lost, so the effective data of 18 patients) as control group. Pelnca therapy group were emergency debridement + Pelnac covering, removed silicone membrane after 4 weeks, continue to change medicine wound every three days,and until the wound epidermis. For five groups of patients with postoperative 12 to 18 months of follow-up and comparative analysis. Using spss19.0 package to statistical analysis.Results: Pelnac treatment group has no major complications postoperatively, effective color and texture ratio is 100%, the time of wound epidermis is 20.95 ±3.78 days, the effective rate of the treatment of pelnac cure finger tip defect was 100%, the sensation recovery in S3- S3 +. Pelnac treatment group compared with other treatment group, scar score lower than other groups(P < 0.05); Length of hospital stay shorter than other groups(P < o. 05); the appearance is better than other groups(P < 0.05); The wound healing time is longer than other groups(P < 0. 05); the functional exercise time earlier than other groups(P < 0. 05); DIP and total TAM is superior to other groups(P < 0. 05). Patients in the hospital expenses, Pelnac higher than that of in situ implant group(P < 0.05), with the island flap group has no obvious statistical significance(P > 0.05), lower than the adjacent flap and abdominal flap group(P < 0.05) Patients with operation time, Pelnac with in situ implant group no significant statistical difference(P > 0.05), short than other treatment group(P < 0.05)Conclusion: 1、Pelnac to repair finger tip defect in hand trauma, simple operation, short operative time, shorter hospitalization time, small trauma, postoperative refers to the look and feel good. 2、 Pelnac treatment an operation is completed, no secondary operation, easy to promote at the basic-level hospitals. 3,、Pelnac treatment after surgery with finger joints can early activities and functional exercise, refers to the activity and function recovered well 4,、Pelnac treatment wound healing cycle is long, aseptic requirement is high, the cost is higher. |
