| Objectives Made investigation and study of community physicians in a district of Hefei.Through the understanding of the national essential medicine system by community physicians,to analyse the influence factors of community physicians on the national medicine system of cognition,in order to make some suggestions and countermeasures for the further improvement of the usage rate of essential medicines.Methods To adopt current situation investigation and design as well as integral sampling method,so as to make investigation at eight community health service centres and two township health centres in District of Hefei,including all on-the-job physicians in health professionals.By using a self-designed questionnaire survey to make investigation,the investigation content mainly includes:the basic information of community physicians,the cognition and usage of the essential drugs by community physicians,such as the cognition and satisfaction of the implementation of the national essential medicine system.The writer adopts an anonymous self-administered questionnaire,liaison officers from the community health service centers and township health centers to issues,explains and recovers.The data are collected and checked,then the data are coded and input Epi Data 3.1 to establish the database,the data for ratio by using the software SPPS13.0,variance analysis,t test and Logistic regression analysis of multiple factors.Results(1)The awareness rate of the community physicians is 90.0% in District of Hefei,but it is low perceptions of community physicians to national essential drugs.(2)Business training and media publicity are the main sources of information forcommunity physicians to know about essential medicines.(3)Multi factor Logistic regression analysis shows that age,personal annual income,professional and technical positions are the main influencing factors of community physicians’ cognition of the national essential medicine system.(4)The choice of the community physicians’ prescription is 93.0% to choose essentialdrugs,the main reasons lie in the higher level officer’s demand,the patient’s demand and the proportion of reimbursement.However,the degree of recognition of the basic medicine efficacy is only 56.7%.(5)76.1% of the community physicians often recommend the use of basic medicines to patients,but it is only 65.2% of the patients are willing to accept the recommended use of basic medicines.(6)38.8% of the community physicians believe that the national essential medicine system can reduce the amount of the prescription,65.7% of the community physicians say the national essential medicine system is conducive to ease the problem in the high cost of getting medical treatment.(7)After the implementation of the basic medicine system,the income of the 10.0% community physicians has increased.(8)the overall evaluation of the national essential medicine system is more positive,but its acceptance and satisfaction are lower,60.2% and 54.2% respectively.Conclusion The concept of essential medicines have been widely known in the community physicians,but most of the community physicians need to know more about the national essential medicine system.Since the implementation of the basic medicine system,it has not achieved significant effect on reducing the amount of the prescription and alleviating the problem in the high cost of getting medical treatment.Performance evaluation mechanism hasnotbeen established for community physicians to generalize and use essential medicines.There is nothing much for the community physicians to accept and satisfy on the national essential medicine systemthe.We should furtherstrengthenthe publicity andeducation of the national essential medicine system.Under this background,we put forward the study on special topic to community physicians at essential medicine.It is important that the study will pay attention to the form,contentandpractical.Besides,it is necessary for residents to improve the cognition of essentialmedicines.Explore theperformance evaluation mechanism in essential medicines,improve the income level of community physicians.These measures are of great significance to ensure the operation of the national essential medicine system. |
