Objective:To investigate the influence of prepregnancy body mass index and body weight changes during pregnancy with pregnancy outcome.Methods:3924 cases of pregnant women with singleton were selected,the incidence of complications in pregnancy(gestational diabetes,gestational hypertension,intrahepatic cholestasis of pregnancy,premature rupture of membranes,placental abruption,postpartum hemorrhage,puerperal infection)and adverse pregnancy outcomes(fetal distress,premature birth,stillbirth),mode of delivery and newborn health(Apgar score,body weight,body length,macrosomia,low birth weight,in NICU)were analyzed,and pregnant women were divided into normal BMI group(n=3126),low BMI groups(n=364)and high BMI groups(n= 434),and incidence of complications in pregnancy,adverse pregnancy outcomes and newborn health situation were contrasted along three groups.then 3126 cases of pregnant women with normal pre pregnancy BIM were divided into normal weight gaining groups(n=2426),low weight gaining groups(n=107)and high weight gaining groups(n=593),incidence of complications in pregnancy,adverse pregnancy outcomes and newborn health situation were contrasted along three groups.Results: In 3924 cases of pregnant women incidence rate of gestational hypertension was 4.46%(175/3924),incidence rate of gestational diabetesmellitus was 10.19%(400/3924),and incidence rate of intrahepatic cholestasis of pregnant was 0.94%(37/3924).Incidence rate of premature rupture of membranes was 4.13%(162/3924),incidence rate of placental abruption was 1.12%(44/3924),incidence rate of fetal distress was 1.73%(68/3924),incidence rate of preterm birth 2.70%(106/3924),stillbirth rate was0.17%(7/3924).incidence rate of cesarean section was 51.38%(2016/3924),incidence rate of postpartum hemorrhage was 3.62%,puerperal infection rate was 0.87%(140/3924).In 3924 newborn,Apgar score was(9.90 ± 0.36),average body weight was(3312 ± 51.75)g and average length(50.56 ±1.24)cm,proportion of macrosomia was 6.75%(265),ratio of low birthweight babies was 1.34%(53/3924),ratio of being NICU is 3.16%(124/3924).Incidence rate of pregnancy hypertension disease was not different in low BMI groups and normal BMI groups(X~2=3.104,p=0.078),and was higher in high BMI groups than in normal BMI groups(X~2=20.394,p=0.000)and low BMI groups(X~2=16.458,p=0.000);Incidence rate of pregnancy hypertension disease was lower in low BMI groups than in normal BMI groups(X~2=31.507,p=0.000)and high BMI groups(X~2=65.195,p=0.000),and was higher in high BMI groups than in normal BMI groups(X~2=30.186,p=0.000);Incidence rate of intrahepatic cholestasis of pregnancy was not different in low BMI groups to normal BMI groups(X~2=0.038,p=0.845)and high BMI groups(X~2=0.214,p=0.643),and was not different in high BMI groups with normal BMI groups(X~2=0.203,p=0.652);Incidence rate of premature rupture of membranes wasnot different in low BMI groups and normal BMI groups(X~2=0.086,p=0.769),and was higher in high BMI groups than in low BMI groups(X~2=40.034,p=0.000)and normal BMI groups(X~2=149.128,p=0.000);Incidence rate of placental abruption was not different in low BMI groups and normal BMI groups(X~2=0.006,p=0.938),and was higher in high BMI groups than in low BMI groups(X~2=5.227,p=0.022)and normal BMI groups(X~2=18.677,p=0.000);Incidence rate of fetal distress not different in low BMI groups and normal BMI groups(X~2=0.387,p=0.531),and higher in high BMI groups than in low BMI groups(X~2=15.296,p=0.000)and normal BMI groups(X~2=82.088,p=0.000);Incidence rate of premature delivery was not different in low BMI groups and normal BMI groups(X~2=0.001,p=0.987),and was higher in high BMI groups than in low BMI groups(X~2=28.680,p=0.000)and normal BMI groups(X~2=130.228,p=0.000);Incidence rate of stillbirth was not different in low BMI groups than in normal BMI groups(X~2=0.487,p=0.485)and in high BMI groups(X~2=0.185,p=0.667),and was not different in normal BMI groups and high BMI groups(X~2=2.510,p=0.113);rate of cesarean section was higher in low BMI groups than in normal BMI groups(X~2=4.887,p=0.027),and not different than in high BMI groups(X~2=1.553,p=0.213),and was higher in high BMI groups than in normal BMI groups(X~2=16.780,p=0.000);incidence rate of postpartum hemorrhage was not different in low BMI groups than in normal BMI groups(X~2=0.710,p=0.400),was not different to high BMI groups(X~2=0.063,p=0.802),and was not different in normal BMI groups and highBMI groups(X~2=1.845,p=0.174);incidence rate of puerperal infection was not different in low BMI groups and normal BMI groups(X~2=0.450,p=0.502),and was lower in low BMI groups than in high BMI groups(c 2=5.477,p=0.019),and was higher in high BMI groups than in normal BMI groups(X~2=31.713,p=0.0000);Score of Apgar was higher in normal BMI groups than in low BMI groups(t=7.146,p=0.014)and high BMI groups(t=7.615,p=0.010),and was not different in low BMI groups and high BMI groups(t=0.064,p=0.816);newborn weight was lower in normal BMI groups than in high BMI groups(t=5.296,p=0.009),was higher than in low BMI groups(t=9.174,p=0.003);Newborn height was not different in normal BMI groups than in low BMI groups(t=2.176,p=0.062)and high BMI groups(t=1.205,p=0.147),and was longer in high BMI groups than in low BMI groups(t=4.286,p=0.029);incidence rate of macrosomia was higher in normal BMI groups than in low BMI groups(X~2=8.296,p=0.004),and was lower than in high BMI groups(X~2=37.277,p=0.000),and incidence rate of macrosomia was higher in high BMI groups than in low BMI groups(X~2=34.083,p=0.000);incidence rate of low birth weight infant was lower in normal BMI groups than in low BMI groups(X~2=32.527,p=0.000),and was not different in normal BMI groups and low BMI groups(X~2=0.098,p=0.754),and was lower in high BMI groups than in low BMI groups(X~2=9.140,p=0.003);rate of being NICU was lower in normal BMI groups than in low BMI groups(X~2=4.193,p=0.041)and high BMI groups(X~2=5.697,p=0.017),and was not different in low BMI groupsand higher BMI groups(X~2=0.012,p=0.911).Incidence rate of pregnancy hypertension disease was not different in low weight gaining groups(X~2=1.520,p=0.218);Incidence rate of stillbirth was not different normal weight gaining groups than in low weight gaining groups(X~2=0.132,p=0.176)and high weight gaining groups((X~2=0.073,p=0.787),rate of cesarean section was not different in normal weight gaining groups than in low weight gaining groups(X~2=0.021,p=0.555),and was higher in high weight gaining groups gaining groups(t=8.076,p=0.008),was lower than in high weight gaining groups(t=9.142,p=0.002);Newborn length was longer in normal weight gaining groups than in low weight gaining groups(t=3.179,p=0.012),was shorter than in high weight gaining groups(t=4.576,p=0.008);Incidence rate of macrosomia was higher in normal weight gaining groups than in low weight gaining groups(X~2=5.604,p=0.018),and was lower than in high weight gaining groups(X~2=42.494,p=0.000),incidence rate of low birth weight infant was not different in normal weight gaining groups and higher weight gaining groups(c 2=0.024,p=0.877),was lower than in low weight gaining groups(X~2=109.119,p=0.000);Rate of being NICU was lower in normal weight gaining groups than in low weight gaining groups(X~2=23.925,p=0.000)and high weight gaining groups(X~2=36.794,p=0.000).Conclusions Abnormalities of pregnancy BMI and weight gaining during pregnancy increases risk of pregnancy complications mid-term and late pregnancy,affect healthy development of fetus,is a reasons of adverse pregnancy outcomes,and affect the health of newborn,Body weight before pregnancy should be control in reasonable level,and pregnant nutrition should be scientific,and pregnancy weight management shoud be taken to decreas pregnancy risk and improve quality of pregnancy.
