| Objective:The aim of this study was to investigate the quality of life of patients after total knee arthroplasty(Total knee arthroplasty,TKA),to analyze the relationship between general demographic characteristics,postoperative knee function,coping style,social support and quality of life,and to explore the factors influencing the quality of life of patients after total knee arthroplasty,then to provide a new theoretical basis for postoperative medical care of patients with total knee arthroplasty with a view to intervening influencing factors,thus obtain better surgical results and maximize the,improvement of total knee arthroplasty after the quality of life of patients.Methods:This study was a cross-sectional study.Using convenience sampling method,a questionnaire survey was conducted from May 2016 to in the Department of orthopedics outpatient department of Qilu Hospital of Shandong University after the total knee replacement surgery for 3 months.A total of 190 questionnaires were issued in this study.The demographic characteristics,HSS scores,coping style self-evaluation(positive,negative),social support and MOS SF-36 quality of life(physical and mental health)of patients after total knee arthroplasty were collected.The relationship between different demographic characteristics and quality of life was analyzed by one-way ANOVA.The relationship between postoperative knee function,coping style,social support and quality of life of the patients were analyzed by Pearson correlation analysis.The factors influencing the quality of life of patients after total knee arthroplasty were analyzed by multiple linear regression(stepwise regression analysis).Results:1.A total of 190 questionnaires were issued in this study.Of these,180 were valid questionnaires and the effective recovery rate was 94.7%.2.The physical health score of the patients after total knee arthroplasty was 300.96 ± 32.01;the mental health score was 296.01 ± 33.46,which were all lower than the normal norm.3.The results showed that the higher the age of the patients,the higher the body mass index(BM1),the greater the economic pressure,the longer the duration of knee osteoarthritis and the lower physical and mental health scores.Higher levels of physical health and mental health scores were higher in patients with higher spouses,spouses,children who had no other medical history.While the family members living with the family had higher physical health scores;living in the nursing home patients with higher mental health score.4.Pearson correlation analysis showed that the HSS score(r=0.908,p<0.001),positive coping style score(r=0.622,p<0.001),social support(r=0.781,p<0.001),mental health(r=0.930,p<0.001)was positively correlated with postoperative physical health.The HSS score(r=0.890,P<0.001),positive coping style score(r=0.666,p<0.001),social support(r=0.836,p<0.001),physical health(r=0.930,p<0.001)was positively correlated with postoperative mental health.The negative coping style was negatively correlated with postoperative body(r=-0.815,p<0.001)and mental health(r=-0.833,p<0.001).5.Multivariate stepwise regression analysis showed that age,the level of education,residential status,the duration of illness,with or without children,HSS score and negative coping style have significant predictive effect on the patient’s physical health;age,the level of education,residential status,HSS score,social support and negative coping style have a significant predictive effect on the patient’s mental health.Conclusions:1.The quality of life of patients after total knee arthroplasty was lower than that of the general population,and this need to pay attention to medical staff.Medical workers should not only pursue the perfect technique of surgical technique,improve the somatic function of patients,but also pay attention to the improvement of postoperative quality of life.2.The factors influencing the physical health dimension of patients with total knee arthroplasty were age,the level of education,residential status,the duration of illness,with or without children,HSS score and negative coping style;influencing factors of mental health dimension were age,the level of education,residential status,HSS score,social support and negative coping style.For these factors,given the patient’s targeted psychological counseling and rehabilitation education,to provideg effective home care and social support,is conducive to maximize the improvement of surgical treatment,improve the quality of life of patients. |
