| Objective: Observation of gestational diabetes mellitus(GDM)correlation between TCM Constitution and insulin resistance and beta cell function,to further explore the clinical significance of the correlation analysis,the distribution characteristics of TCM constitutions,and explore the methods to improve the biased constitution,to provide effective reference for the prevention and treatment of GDM with traditional Chinese medicine.Methods: Collection in November 2014 to October 2016 to our hospital underwent an oral glucose tolerance test(OGTT)of pregnant women,screening with GDM were included in 212 cases as the research object,and randomly selected 210 cases of normal glucose tolerance group(NGT)as the control group.The investigation of constitutional types of Chinese medicine and Chinese Medicine,fill in the basic information questionnaire,detection of the completion of data collection and related indicators.Calculate HOMA-IR,HOMA-β,application SPSS17.0 to establish a database for statistical analysis.Study the GDM crowd of TCM Constitution Types distribution,and to explore the correlation between TCM Constitution Types and HOMA-IR,HOMA-β.Results:(1)GDM group and NGT group,the blood glucose,insulin,HOMA-IR,HOMA-βdifferences were statistically significant(P<0.05);the blood glucose and fasting insulin were higher in GDM group OGTT,delayed peak postprandial insulin secretion.The levels of TG,TC and LDL-C in serum of GDM group were higher than those in NGT group,the difference was statistically significant(P<0.05),HDL-C level was not statistically significant(P>0.05).(2)212 cases of GDM in the distribution of TCM Constitution for phlegm wet mass 60 cases(28.3%),49 cases of damp and hot(23.1%),24(11.3%)cases of Yang deficiency and qi deficiency in 20 cases(9.4%),and mass of 32 cases(15.1%),quality of qi depression in 10 cases(4.7%),9 cases of yin deficiency(4.2%)blood stasis,quality in 5 cases(2.4%),the intrinsic quality of 3 cases(1.4%),in addition to gentleness,with phlegm dampness and dampness heat,Yang deficiency,qi deficiency is the most common.Two groups of traditional Chinese medicine constitution type distribution comparison: the two groups of TCM constitution type overall difference(P<0.05).(3)The comparison of the GDM group of TCM Constitution Types in HOMA-IR and HOMA-β level: there is statistical significance in different types of TCM Constitution of HOMA-IR and HOMA-β levels between the two groups were different(P<0.05),the HOMA-IR level: phlegm dampness>damp heat>Yang deficiency>yin deficiency> Qi deficiency>blood stasis>qi stagnation>the intrinsic quality>quality;HOMA-β level: phlegm dampness<Yang deficiency<blood stasis<Yin deficiency<qi stagnation was the intrinsic quality of gentleness.(4)Comparison of GDM group of TCM constitution classification and pre pregnancy weight pre pregnancy BMI: there was statistical significance before the physical type of BMI levels between the two groups were different(P< 0.05),the phlegm,Yang deficiency,damp heat,blood stasis,intrinsic quality,qi stagnation,qi deficiency,and quality,Yin deficiency.(5)The correlation between HOMA-IR,HOMA-β and other indicators of relevance: by Pearson analysis,HOMA-IR and Pre-pregnancy BMI,FPG,FINS,TG were positive correlation,through correlation coefficient respectively(r=0.272,r=0.237,r=0.453,r=0.292),the significant test(bilateral)P<0.01.HOMA-β was negatively correlated with pre-pregnancy BMI,FPG,FINS,TG,HOMA-IR,the correlation coefficients respectively(r=-0.306,r=-0.196,r=-0.167,r=-0.259,r=-0.327),significant test(bilateral)P < 0.01 or P < 0.05.Conclusion:(1)GDM group with phlegm dampness,damp heat,yang deficiency,qi deficiency is the common type of TCM constitution,in which phlegm dampness,damp heat is the most common.(2)Between the different physical HOMA-IR and HOMA-β have obvious difference,pre-pregnancy BMI,FPG,TG through to GDM group for various types of physical distribution and HOMA-IR,HOMA-β have obviously influence. |
