ObjectiveTo observe the clinical effect of surgical treatment for chronic lateral instability of the ankle. MethodAll the 130 patients with chronic lateral instability of the ankle who had been accept operation at our department from October,2012 to October,2015. Those include 87 males and 43 females aged from 20 to 54 years(34.6 years at average).All patients had unilateral sprains with 68 left injury ankles and 62 right ones. There is 3 months to 7 years before they visit the doctor{(4.63 ±1.02) years at average}. We choose surgical options according to the patient’s situation. Regularly arthroscopy is adopted before ligament reconstruction such as micro fracture repair surgery to treat cartilage injury. We adopt resection to treat ankle synovium cartilage tumour disease, clear ankle loose bodies, ankle osteoarthritis and synovial membrane hyperemia and edema. We adopt astragalus cartilage transplantation surgery to treat large pieces of cartilage defects associated with the subchondral bone necrosis. After arthroscopic surgery, We cut a "L" type small incision the in the lateral of ankle to expose to injury ankle lateral ligament.We choose surgery options according to the patient’s situation of injury ligament. We use AOFAS rating system to evaluate ankle preoperative and postoperative function. Then we adopt statistical analysis. ResultThe AOFAS score of all patients postoperation is 85.87±7.45, compared to 66.11±5.12 preoperation.The statistical difference is significant(t = 9.665, P<0.05). The AOFAS score of patients with repairing anterior talofibular ligament and calcaneofibular ligament is 86.34±7.69, significantly more than the 65.81±5.31 preoperation.The statistical difference is significant(t = 9.362, P <0.05). The AOFAS score of those with repairing anterior talofibular ligament was 85.19±8.35,more than 66.37±6.23 preoperation. The statistical difference is significant(t = 10.225, P < 0.05).The AOFAS score of those with repairing calcaneofibular ligament was 86.16±7.15, more than the preoperative 67.57±5.18.The statistical difference is significant(t = 9.889, P < 0.05).. ConclusionSurgical treatment for chronic lateral instability of ankle can mprove the stability and release the pain, and the trauma of the surgical is small.The prognosis effect is good.
