To want to do for prevention and control, we must understand the current medical institutions to control the infection status, medical institutions, hospital infection, to understand the effect of the evaluation of medical institutions of the cleaning, disinfection and sterilization quality and hospital sterilization and infection control measures, in order to provide the basis for disinfection of hospital infection control and related policies and regulations, standards and revision.2012-2015 by infection in different areas of Shandong Province at different levels of medical institutions control influence factors of monitoring and investigation, understand the medical institutions of the cleaning, disinfection and sterilization quality and disinfection in the hospital environment, a comprehensive analysis of Shandong Province all types of medical institutions disinfection effect, find out related factors, so as to reduce the nosocomial infection incidence of provide the basis, In order to promote the improvement of the quality of disinfection, reduce the occurrence of iatrogenic infection.MethodsAccording to " technical standard for disinfection", "endoscopic operation technique of the cleaning and disinfection specification", and other provisions, from 2013-2015 years,the Infection control influence factors of 94 medical hospitals in 11 city of Shandong Province were monitored and investigated. Monitoring and investigation contents include nosocomial infection, medical equipment cleaning and disinfection and sterilization effect, medical staff hand hygiene and compliance, hospital environment, microbial contamination and disinfection effect.Results94 medical institutions In Shandong province are monitored from 2013 to 2015。1. The number of medical devices detected by ATP bioluminescence method in 2013-2015 was 235,300 and 320, respectively, and the qualified rate was 36.6%, 55.7% and 68.1%, respectively. There is an upward trend in cleaning effect qualified rate of medical equipment.2. According to survey on endoscopic workload in 2015,it showed that the survey day clinics and quarter maximum treatment capacity in the tertiary hospital is greater than the second level hospital; single frequency of use of endoscopic results show that secondary hospitals more than tertiary hospital.In 2013 to 2015,709 endoscopy were detected, the overall pass rate was 79.7%;. In 2014 and 2015, the pass rate of rinse water for endoscope disinfection in the tertiary hospital and the second level hospital were 71.4%,72.9%.3.2013 -2015 medical staff hand hygiene compliance rate was 40.0%,31.9%, 27.2% respectively; the qualified rate of surgical hand disinfection was 92.6%,93.3%, 91.3%, hygienic hand disinfection qualified rate was 67.8%,80.4% and 76.4%; the qualified rate of hand washing water was 72.2% in 2014,2015.4.2013 -2015, bacterial sedimentation assay shows that the qualified rate of operation room was 61.1%,79.5% and 82.5% respectively, and with planktonic bacteria monitoring, the qualified rate of operation room was 66.7%,83% and 100% respectively. The results of floating bacteria P50 in grade Ⅰ, grade Ⅲ and general operation room were 18.8 cfu/m3,76 cfu/m3,342 cfu/m3, respectively.5. In 2013-2015,the disinfection qualified rate of general object surface in the key departments were 76.1%,80.3% respectively; qualified rate of class Ⅱ, Ⅲ/Ⅳ environment object surface before disinfection were 72.1% and 78.9%; and the qualified rate after disinfection were 79.0% and 81.5% respectively.6.2013 -2015,1348 medical water in the department of stomatology were monitored, the overall pass rate of 42.5%, the pass rates of mobile water, washing water, source water, water in storage bottles were 32.5%,43.9%,48.6%,61%. The results show that the water pollution problem is still serious in department of stomatology.7. Total of 120 sewage after disinfection from medical institutions were monitored, the overall pass rate was 66.7%, and pass rate was 50%,55.4%,84.8% in 2013-2015.ConclusionFrom the monitoring results, we can see that current medical institutions in cleaning medical instruments, air quality, environment object surface, hand hygiene there are many risk factors, especially in compliance and oral water disinfection and sterilization effect and Standard Specification for certain gaps and problems still exist, still need to further strengthen the monitoring and management. |
