Research In The Academic Thoughts And Clinical Experience About Auricular Points Of Professor Zhudan

Objective:To collect and summarize the academic ideas and clinical experience of the Professor Zhudan. Observe the clinical effect of auricular application for treatment of neck and shoulder’s muscle injury(Cervical Spondylotic Radiculopathy,CSR). Do every effort to retain the technology and experiences of ear diagnosis and auricular therapy from Professor Zhu. Based on this deduction, put forward the inference and innovation, in order to transmit the essence of academic thoughts of Professor Zhu and make progress.Methods:(一) With the method of literature research and the development of the theory of the origin of the ear and ear system formation; Through the way to study the teacher’s books, articles, and interviews with Professor zhu directly, research the source of the academic theory about acupuncture, especially the auricle diagnosis and the auricular therapy; With follow-up, through analysis of typical medical record, summarize the clinical experience in diagnosis and treatment of auricle therapy technology from Profeesor zhu.(二) On the basis of the experiences of professor Zhu, combined with the understanding and recognition of author, infer that the effect of treatment of auricular application is very well for disase of Poor circulation of Qi in meridians. Guided by Zhu, independently design a prospective randomized controlled clinical study, observed the effect of auricular application treatment for muscle injury of neck and shoulder(cervical spondylotic radiculopathy).100 patients of CSR were randomly divided into observation group and control group, two groups include 50 cases each. Control group accepted the electric acupuncture treatment only.Besides it,observation group accepted auricular application therapy more.Scored the symptom of cervical spondylosis with TianZhongjingjiu symptom scale, and evaluated the effect based on the observation of disease diagnosis curative effection of traditional Chinese medicine standard. By analyzing the result and data of this clinical randomized controlled study, refined the essence of the thoughts from professor Zhu about the clinical experience of auricular therapy.Results:(一) Summarizes the academic origin of Professor Zhu:her academic origin lies in ancient books, lapping, combined with clinical study and thinking, good at summarizing. Professor Zhu graduated from health colleage of 7th military medical university.Then she studied systematicaly the traditional Chinese medicine,acupuncture,auricular therapy,and clinic skills in acupuncture classes of health bureau in Chongqing, Chongqing institute of traditional Chinese medicine orderly.Then she continue study in acupuncture research in stitute of China academy of traditional Chinese medicine, Beijing in stitute for advanced and further study of acupuncture. During the reading time, she studied hard, not only diligent study the theory of traditional Chinese medicine, reading extensively medical books and classics from past famous doctor,but also keep practicing the technical operations such as acupuncture and auricular therapy day and night.All this efforts laid a solid theoretical and clinic basis for her a chievements now.Professor Zhu had ever learn acupuncture from famous acupuncturist Liuliwu, and internal medical theory of TCM from noted doctor of TCM Lizhuan.She further broaden the vision and accumulated experience,she well inherited two teacher’s academic thoughts and clinical experience. In addition, she also pay attention to progress and dynamic of research fromhome and aboad about auricular therapy.She insisted on reading literatures, and attended academic conferences, unceasingly absorb the most advanced knowlege and technology of diagnosis and treatment of the auricular points. Through unremitting efforts, with full use of the advantages, combined with their decades of clinical experience,she formed her own unique academic experience.(二) To summarize the academic thoughts of Professor Zhu:The two ears are one little "taiji",the same of the body.(三) To summarize the clinical experience of Professor Zhu about auricle diagnosis and therapy.1. The auricular diagnosis experience of Professor Zhu:(1)The main points of auricular diagnosis of Professor Zhu maybe sumarized to four words:"observe", "search", "touch" and "analyze"."observe", required to look at the appearance of the auricle, in clude appearance and skin color, "observe" is the foundation, is the the basic way and requirement of auricular diagnosis method. "Search" means looking for auricular "positive sign", is the specific operation method and purpose of "observe". "Touch" is palpation for positive sign, and use the thumb and index finger to palpate auricle.It is the premise and basis for "analyze". "Analyze" is to carefully identify the characteristics of positive sign, including locations,properties, morphology, size, hardness, movable degree, whether the edges is tidiness and tenderness. "Analyze" is to identify and determine the final results for diagnosis.(2) The main contents of auricular diagnosis are three parts:"color", "form" and "vein".Color refers to the changes of pinna skin color, the main diagnostic information has been obtained through observati on. Form on the skin is to look at the auricle and the appearance changement of positive signs, mainly through searching and touch, to obtain diagnostic information. Vein of auricle, especially the change of the blood capillary.The first-hand materials are got by observation.(3) Advocate "syndrome differentiation of Yin and Yang" with the clinical information:all the symptoms belongs to "yang",such as pinna bright, red color, luster, auricle generous, is visible on the auricle steatorrhea, uplift, vein expansion, red papules etc. Instead, symptoms belongs to "yin", such as auricle colour and lustre is dim, or pale gray, lacklustre, auricle small, thin ears on a dry, depression, dark gray papules etc.(4) About the operation of auricular diagnosis, she put forward the following specific clinical requirements:"skilled", "avoid", "bright", "exclude" and "analysis". "Skilled" point to the requirement for doctor to be familiar with the anatomic location of auricle, concrete 1 ocation and distribution of auricular points. "Avoid" point to avoid brush,massage, or bath auricle, lest affect original color and shape of auricle. "Bright" is the auricle diagnosis must be done under the bright environment. "Exclude",should be careful to eliminate false diagnosis, if necessary in combination with other diagnosis methods, instruments and general inspection. "Analysis" means analyze the specific information, and think about the different time,location and diferent nature of people fully.(5) Experiences of clinical tumor characteristics from professor Zhu. She thinks that auricle often will appear the characteristic performance when tumour occurs, such as a small black dots pigmentation, cartilage proliferation, tenderness ect. in the pinna tumour-specific area.2. The academic experience of auricular therapy from professor Zhu:(1) There are "three-step" in her clinic experience:the first step is to put pressure carefully prepared before, including the dialectical prescription.This is the basic premise for clinical curative effect; Secondly,to find accurate location.This an important guarantee for clinical curative effect; Thirdly,clear orders after the therapy.Which is promoting factors to obtain the clinical curative effect.(2)Professor Zhu attach great importence to the formula, in clinical, pay attention to targeted for different disease with auricular point, the main point features inculud choosing main point by location of disease, choosing the minor points by nature of disease, and increasing or decreasing points by specific symptoms.(四) The clinical randomized controlled experiments to observe the curative effect of auricular application therapy on cervical spondylotic radiculopathy(CSR):observation group and control group after treatment scores were better than those before treatment (P<0. 05); Score difference before and after treatment of observation group was obviously higher than that of control group (P< 0.05); Observation group cure rate was significantly higher than that of control group (P<0.01).Conclusions:Professor Zhu is the notable doctor in Chongqing, the standing committee member of auricular points committee of China Institute of Acupuncture and Moxibustion,the former head of the auricular diagnosis group,the chairman of auricular points com mittee of Chongqing,and famous expert of auricular therapy in China,the coordinate guider with "master of TCM" Chen ZhiLong.She is noble ethics,thoughtful,superb techniques,theoretical knowledge,and rich clinical experience.The results of RCT of auricular therapy for CSR confirm the validity of the clinical experience from professor Zhu,and verified my conclusion that it can treat the symptoms of poor circulation of meridians.It is showed that the theory of professor Zhu is open,can achieve the innovation and development. Her academic theory and clinic technology are unique,and added a very valuable asset to science of acupuncture and moxibustion,meaningful leaning,inherit and carry forwad.
