Part Ⅰ The Effect Of Ischemic Preconditioning Training On NIHSS And Serum Cys C Level In Patients With Acute Cerebral Infarction And Correlation Analysis Of NIHSS And Serum Cys C LevelObjective To investigate the effect of ischemic preconditioning training on NIHSS and serum Cys C level in patients with acute cerebral infarction,and to explore the correlation between NIHSS and serum Cys C level.Methods A total of 82 patients with acute cerebral infarction in our hospital from March 2012 to December 2014 were selected.They were randomly divided into observation group and control group according to the number table,41 cases in each group.Patients in the control group were received conventional treatment,while patients in the observation group were given ischemic preconditioning training on the uplink limb based on the control group.They were given continuous treatment for 14 d.The NIHSS scores,Cys C,CRP,Scr levels were recorded before and after treatment.NIHSS score:(1)basic healing: score after treatment to reduce 90% or higher,(2)remarkable progress: score decreased by46% ~ 46%,and(3)progress: score decreased by 18% ~ 18%,and(4)no change:reduction or increase 17% or less,deterioration of(5): score increase 18% or higher.With Cys C,CRP,Scr level change,Cys C relevance were analyzed by using Spearman correlation analysis.Results The symptomatic improvement and metabolism improving,infarct volume reduction of observation group was 82.93%,,which was significantly higher than in control group 63.41%,(χ2=3.9758,P <0.05).The NIHSS scores in two groups after treatment were significantly lower than that before treatment.NIHSS scores after treatment in the observation group was significantly lower than the control group(P <0.05);after treatment Cys C,CRP,Scr were significantly lower than that before treatment in two groups(P <0.05),after treatment Cys C,CRP,Scr in the observation group were significantly lower than in the control group(P <0.05).NIHSS score was positively correlated to serum Cys C level(r = 0.579,P = 0.00).Conclusions The results show that Cys C level associates with acute cerebral disease.The ischemic preconditioning training can improve neurological symptoms in patients with acute cerebral infarction;reduce serum Cys C levels in patients,which will help to improve the prognosis of patients.Part Ⅱ The Study On The Serum Ferritin,Fibrinogen And D-Dimer Levels In Patients With Small Vessel Disease Department Of NeurologyObjective To explore the correlation of serum ferritin(SF),fibrinogen(FIB)and D-Dimer(D-D)levels in patients with small vessel disease(SVD).Methods 359 patients with SVD,including 186 patients with lacunar infarction(LI)and173 patients with leukoaraiosis(LA),and 176 healthy cases were enrolled in our study.All the patients were from the Department of Neurology,First Affiliated Hospital of Anhui Medical University between Jan.2010 and Dec.2012.And the health controls were from the health check-up centre.The SF,FIB and D-D concentrations were measured by radioimmunoassay method and Clauss method respectively.Result The serum levels of SF in patients with SVD(171.82±156.28ug/l)was significantly higher than those in healthy control group(100.37±70.11ug/l)(P<0.01).Meanwhile,the level of FIB and D-D were also higher in SVD compared to controls(3.27±0.93 VS.2.97±0.88g/L,1.10±1.06 VS.0.83±0.55ug/ml).Moreover,the LA subgroup presented a more significant change when compared to the LI subgroup whether in SF,FIB or D-D.Multivariate regression analysis revealed that SF and FIB were independent variables predicting the occurence of SVD.When the subjects were categorized into quartiles by SF and FIB,by comparison to the reference quartile.he odds ratio(95% CI)for SVD patients in the higher quartile(SF>151.16ug/l,D-D>1.02ug/ml)were 3.444(95%CI=2.097-5.158,P < 0.001)and 1.767(95%CI= 1.114-2.803,P=0.016)respectively after multivariable adjustment.Conclusion The increased SF,FIB and D-D was closely related with SVD,exclusion of SF,FIB and D-D might come to be attractive approaches to prevent the progress of SVD.
