Clinical Application And Discussion Of Day Surgery On Vocal Cord Polyp With Different Surgical Methods

Objective:Analyzing the clinical data of 160 patients diagnosed with vocal cord polyp and undergoing the day surgery,the paper discusses adopting the suitable surgical methods to treat a variety of vocal cord polyps,analyzes the reasons why the patent postpones the discharge from the hospital and improves the work of day surgery.Methods:Selecting 160 patients treated in our hospital from October 2015 to December 2016,diagnosed with vocal cord polyp and undergoing the day surgery as the observation group among which 80 patients undergo CO2 laser resection surgery and the rest have laryngeal microsurgery with instrument,the paper chooses 30 family members of the patients without voice disorder as the control group,conducts subjective and objective analysis of the voice of the observation group before and after the surgery as well as observes the curative effect.Results:Among the patients with vocal cord polyp in the observation group and control group,the number and age of men and women have no statistical differences(p>0.05).CO2 laser surgery group 1 month before the surgery is compared with the CO2 laser surgery group 1 month after the surgery and the results show no statistical differences of TVH,VHI-F,VHI-P,VHI-E,MPT(p>0.05).Simple and routine polyp surgery group,simple CO2 laser polyp surgery group 1 month after the surgery and the control group are compared with one another as well as the results show no statistical differences(p>0.05).Complicated CO2 laser polyp surgery group 1 month after the surgery is compared with the control group and the results show no statistical differences(p>0.05).Subglottic routine polyp surgery group is compared with the subglottic CO2 laser polyp surgery group 1 month after the surgery and the results show no statistical differences(p>0.05).Anterior-commissure CO2 laser polyp surgery group 1 month after the surgery is compared with the control group and the results show no statistical differences(p>0.05).Routine surgery group 1 month after the surgery is compared with the control group and the results show that VHI-P has statistical differences(p<0.05).Complicated and routine polyp surgery group 1 month after the surgery is compared with the control group and complicated CO2 laser polyp surgery group respectively as well as the results show that VHI-P,VHI-E and MPT have statistical differences(p<0.05).The control group is compared with subglottic routine polyp surgery group 1 month after the surgery and subglottic CO2 laser polyp surgery group 1 month after the surgery respectively as well as the results show that MPT has statistical differences(p<0.05).Anterior-commissure routine polyp surgery group 1 month after the surgery is compared with the control group and the results show that VHI-P,VHI-E and MPT have statistical differences(p<0.05).Anterior-commissure routine polyp surgery group is compared with anterior-commissure CO2 laser polyp surgery group 1 month after the surgery and the results show that VHI-E and MPT have statistical differences(p<0.05).CO2 laser group is compared with the evaluation of fibrolaryngoscope of the routine group 1 month after the surgery and the results show the effective rate of CO2 laser group after the surgery is 97.50%and the effective rate of routine group after the surgery is 93.75%.The comparison between the efficiency of two groups has no statistical differences(p>0.05).The comparison between the complication of CO2 laser group and routine group after the surgery has no statistical differences(p>0.05).3 patients fail to undergo the day surgery due to high blood pressure and postpone the discharge from the hospital.3 patients have the postoperation pain.1 patient suffers from nausea and vomiting.Conclusion:1.Based on the evaluation scores of the subjective and objective voice of the patients and the fibrolaryngoscope,results show that the resection surgeries of vocal cord polyp with CO2 laser and routine laryngoscope are effective surgical methods to treat the vocal cord polyp as well as the effective rate of the two methods are similar.2.CO2 laser and routine laryngoscope can effectively treat the simple vocal cord polyp and have the similar curative effect.In terms of complicated vocal cord polyp,the effect of CO2 laser surgery is better than routine laryngoscope surgery.For the anterior-commissure polyp,the effect of CO2 laser surgery is better than routine laryngoscope surgery.Two methods can effectively treat the subglottic polyp.3.Inquire about the details of patients' medical history,fully communicate with the patients before the surgery,properly use the narcotic and the patients can be discharged from hospital on time.
