| Purpose: To observe the clinical efficacy and safety of Sanren Decoction with modified in the treatment of obese type 2 diabetes mellitus with diabetic nephropathy and to disguss the mechanism of action.Methods: Adopting a randomized and parallel-setups trial method,60 patients met the inclusion criteria were randomly divided into control group and treatment group,each with 30 cases.The control group was treated with routine Western Medicine,and the treatment group based on routine treatment of Western Medicine combined with Sanren decoction with modified,each of them treated for 4 weeks.Observe the changes in the two groups before and after treatment on symptom scores,body mass index,blood glucose,glycosylated hemoglobin,serum insulin,blood lipid,urinary protein,renal function,serum resistin,urine and liver type fatty acid binding protein indicators,and using SPSS 19.0 software for statistical analysis.Results:(1)The total effective rate of the treatment group was 90.0%,66.7%in control group,the total effective rate of the treatment group was better than the control group,and the difference between the two groups was statistically significant(P<0.05).(2)The effect on improving syndrome score:Two groups of patients after treatment compared with before treatment,the syndrome score decreased significantly(P<0.01);and the treatment group was better than the control group compared with the control group after treatment(P<0.05).(3)The effect on improving body mass index:The patients in the treatment group compared with before treatment,the body mass index was significantly improved(P<0.05);and there were no significant changes in body weight index in the control group before treatment(P>0.05);compared with the control group after treatment,the treatment group were better than those of the BMI control group(P<0.05).(4)Hypoglycemic effect: The two groups of patients with fasting plasma glucose,postprandial 2 hours blood glucose were significantly reduced after treatment,compared with before treatment have obvious difference(P<0.01),and the effect of treatment group was better than the control group after the treatment between the two groups(P<0.05).(5)Lipid-lowering effect:The two groups of patients with TG,TC and LDL-C decreased compared with before treatment,HDL-C increased compared with before treatment(P<0.01),and the effect of treatment group was better than the control group after the treatment between the two groups(P<0.05).(6)Effect of improving islet function:The fasting serum C-peptide levels in the two groups were higher than those before treatment(P<0.01),and the curative effect of treatment group was better than that of control group after treatment(P<0.05).(7)Reduce the role of urinary protein:The 24 h MAU and 24 h U-TP in two groups of patients were lower than those before treatment(P<0.01),and the curative effect of the treatment group was better than that of the control group after treatment(P<0.05).(8)Effect of improving the renal function:The two groups of patients with Scr and BUN decreased compared with before treatment,while Ccr increased compared with before treatment(P<0.01),and the effect of treatment group was better than the control group after the treatment between the two groups(P<0.05).(9)Effect of improving serum resistin and liver fatty acid binding protein:The two groups of patients with serum resistin and urine L-FABP decreased compared with before treatment(P<0.01),and the effect of treatment group was better than the control group after treatment between the two groups(P<0.05).Conclusion:(1)Sanren Decoction with modified in the treatment of of obese type 2 diabetes mellitus with diabetic nephropathy was safe and effective and worthy for further study.(2)Sanren Decoction with modified in the treatment of of obese type 2 diabetes mellitus with diabetic nephropathy can effectively improve the symptoms,and have better control effect on syndrome score,body mass index,blood glucose and blood fat,glycosylated hemoglobin,fasting serum C-peptide,urine protein,renal function,serum resistin,urine and liver type fatty acid binding protein.(3)Speculated that the mechanism of the Sanren decoction may lie in the insulin signaling pathway in the process of regulate glucolipid metabolism abnormal expression,Inhibiting insulin resistance-related signal pathway activation which leaded by oxygen free radical and inflammatory factor. |
