The Effect Comparison Of Treatment Of Humerus Surgical Neck Fracture By Small Splint And Locking Plate

Objective: to provide references for the clinical treatment of the proximal humeral fracture by comparing the efficacy of treating humerus surgical neck fracture which is under the treatment of manipulative reduction small splint fixation and locking proximal humeral plate fixation.Method: retrospectively analyzing 47 patients with fracture of Surgical neck of humerus who were selected according to inclusion and exclusion criteria,from March 2014 to March 2016,in Hubei Provincial Traditional Chinese Medical Hospital’s.The selected patients are divided into manipulative reduction small splint fixation and locking proximal humeral plate fixation group.Postoperative positive exercise associated with TCM dialectical internal treatment and external treatment of fumigating and washing with TCM were applied in these patients.A series of indexes which include the good results of reduction,the clinic healing,the assessment of shoulders function at 1 month,3 months,and half a year after treatment,and also the satisfactory rate of the assessment of shoulders function in the year after having surgery were observed and compared.Data analysis was performed by using SPSS19.0,the inspection level all should be α=0.05,P<0.05 was considered statistically significant.Results: All 47 patients were followed-up for 6 months averagely.All cases received quite satisfactory score of the shoulder joint function assessment.Among them,there are one cases of tension vesicle and one case of pressure ulcers in manipulative reduction small splint fixation group,which had been cured by timely dealing.And locking proximal humeral plate fixation group were cured without surgical incision infection and failure in internal fixation.Locking plate fixation group have advantages to small splint fixation group in the good rate for reduction(P<0.05);The clinic healing time of fracture small splint fixation group was obviously shorter than that of locking plate fixation group(P<0.05).For the shoulder joint function assessment.Locking plate fixation group is higher than small splint fixation group(P<0.05)after 1 months treatment;But after 3 months treatment,there are no significant difference between the two groups,not significant(P>0.05);And after half a year treatment,small splint fixation group,good rateof 81.48% fine with locking plate fixation group no significantdifference between the rate of 85%,not significant(P>0.05).Conclusion: Both groups have good clinical treatment effect.Locking proximal humeral plate fixation is superior to manipulative reduction small splint fixation in the aspect of fracture reduction and early functional rehabilitation,which is opposite in the aspect of clinical time.Although small splint fixation group is worse than Locking plate fixation group in the shoulder joint function assessment after 1 months treatment,There is still no obvious difference in the shoulder joint function assessment and satisfactory rate of function assessment after 3 months and half a year’s treatment.The treatment of manipulative reduction small splint fixation also can result in the same effects as surgical operation with proper post operation function training and combined internal and external therapy for its advantages of small wounds and fast operation wound healing.And the effect is exact,so as to be worthy of clinical application.
