Applied Analysis Of Optimized Drug Therapy Of Heart Failure In The Elderly People

Background and objective:In the era of aging of the population in the worldwide,heart failure is widely common,the morbidity increased with age.Heart failure has become a heavy medical burden in a developed country,and gradually in developing countries.In the recent 30 years,an indisputable fact is the progress of treatment of cardiovascular diseases,especially the treatment progress of ST-segment elevation myocardial infarction and hypertension makes the number of non-aged people with these disease into old age increase so that the number of population of elderly with heart failure is further enlarged.In this background,risk factors and complications of cardiovascular disease eg.type 2 diabetes mellitus?obesity?hyperlipemia and oxidative stress accelerate the development of heart failure ? end-stage heart disease and death.The current Management Guidelines of heart failure is thought that heart failure is a result that a variety of structural and functional cardiac disease causes impaired ventricular filling and ejection.Comprehensive therapy is recommended in the current Management Guidelines of heart failure which is published by the American college of cardiology,foundation/American heart association,European Society of Cardiology and the Chinese medical association cardiovascular epidemiology branch.It is important that these guidelines are based on data of randomized clinical trial of drug therapy of cardiovascular disease,but the research subjects of these trials are mostly the non-elderly male patients.Although the number of population suffering from cardiovascular disease is great,data of RCT especially for the treatment of heart failure or heart failure are lack.In spite of limitation of RCT and the above concerns,every guide provides the recommendation on drug treatment and non-drug treatment of non-aged and aged patients with heart failure.Apart from the warning of aging,its treatment has no significant difference.The study aims to evaluate the effect of optimized drug therapy of elderly patients with heart failure analysis,the basis for establishing optimized heart failure therapy system.Research Methods:Selection in July 2014 to July 2016 in our hospital for diagnosis and treatment of 45 cases of elderly people with chronic heart failure,optimized heart failure treatment,before treatment and achieve drug dose 3 months respectively after a cardiac ultrasound measurement of left ventricular ejection fraction log data,and USES MLHFQ cardiac insufficiency scale to evaluate the clinical effect on patients,statistical analysis.Results:Optimized drug therapy before and after 45 cases of senile heart failure patients with systolic blood pressure(SBP),diastolic blood pressure(SDP),heart rate,NYHA classification,echocardiographic indexes,left ventricular end-diastolic Canon(LVEDd)were significantly decreased significantly,echocardiogram indexes: left ventricular ejection fraction increased significantly,MLHFQ composite scores,body areas scoring,mood scores were significantly lower,the differences were statistically significant(P < 0.05).After optimal treatment for six months,there were 19 patients(42.22%)whose treatment results was excellent,there were 20 patients(44.44%)whose treatment results was effective,and there were 6 patients(13.33%)whose treatment results was ineffective.The results of subgroups analysis showed that clincial results of patients with different years old,gender and category of heart failure were not signifcantly different(P>0.05),but clincial results of patients with different New York Heart Academy grade was signifcantly different(P<0.05).Conclusion:The optimization of senile heart failure treatment can make the elderly people with heart failure cardiac function improved significantly,improve the quality of life,has the application value.Years old,gender and category of heart failure were not influence factors to the clincal efficacy of heart failure in older patients,but the New York Heart Academy grade was the influence factor to the clincal efficacy of heart failure in older patients.
