Levonorgestrel-releasing Intrauterine System Versus Oral Progestins For Endometrial Hyperplasia:A Meta-analysis

【objective】: Through the meta-analysis to asesee the efficacy and safety about Levonorgestrelreleasing intrauterine system versus oral progestins for endometrial hyperplasia.Providing conservative treatment evidence for the endometrial hyperplasia patients.【Materials and methods】: After determining the retrieval words and retrieval stategy,computer retrieval of relevant databases,Pubmed,Medline,Embase,CBM,CNKI,Wanfang,VIP,Chinese biomedical literature database,retrieval time in 2000.1-2016.12,language restrictions for Chinese and English.the research are randomized controlled study and case control study about levonorgestrel-releasing intrauterine system versus oral progestins for endometrial hyperplasia.after the quality evaluation of the literature of all incuded,extract the data.stata 12.1 software was used for statistical analysis.【Results】: A total of 12 articles were included in the literature,including 8 in English and 4 in Chinese,in the 12 aeticles,there are 8 randomized controlled studys and 4 case control studys,a total of 1068 cases of patients,including 504 cases in the group of Levonorgestrel-releasing intrauterine system and 564 cases in the group of oral progestins.there are 7 articles putted endometrial hyperplasia with non-atypical as the research object,giving treatment of levonorgestrel-releasing intrauterine system versus oral progestins,follow-up of 3,6,12,24 months,meta-analysis separately,the results show that LNG-IUS was more effective than oral progestins after 3 months(RR=1.20,95% CI:1.07~1.34,z=3.13,P=0.002);LNG-IUS was more effective than oral progestins after 6 months(RR=1.34,95% CI:1.20~1.50,z=5.20,P=0.000);LNG-IUS was more effective than oral progestins after 12 months(RR=1.36,95% CI:1.20~1.54,z=4.80,P=0.000);LNG-IUS was more effective than oral progestins after 24 months(RR=1.50,95% CI:1.23~1.83,z=4.05,P=0.000)there are 3 articles putted endometrial hyperplasia with non-atypical and atypical endometrial hyperplasia as the research object,giving treatment of levonorgestrel-releasing intrauterine system versus oral progestins,follow-up seveal months,meta-analysis,the results show that LNG-IUS was more effective than oral progestins(RR=1.45,95% CI:1.25~1.68,z=4.94,P=0.000).there are 4 articles putted endometrial hyperplasia with non-atypical and atypical endometrial hyperplasia as the research object,giving treatment of levonorgestrel-releasing intrauterine system versus oral progestins,follow-up seveal months,meta-analysis,the results show that LNG-IUS was more effective than oral progestins(RR=1.33,95% CI: 1.18~1.50,z=4.76,P=0.000).【Conclusion】: Levonorgestrel-releasing intrauterine system was more effective than oral progestins for endometrial hyperplasia.LNG-IUS may is the fist choice for conservative treatment of endometrial hyperplasia.
