| Objective:Based on the clinical data of 60 cases of vocal cord polyp patients were retrospectively studied,focusing on the voice handicap index scale(the voice handicap index,VHl),voice acoustic analysis(acoustic analysis),range map(Voice range profile,VRP)trends and correlation of sound quality changes before and after the change in patients with surgery treatment.Methods:from April 2015 to April 2016 due to vocal cord polyps treated in our department were diagnosed according to "clinical guidelines-Department of ENT section"(Chinese Medical Association,people’s Medical Publishing House),"clinical specifications-Department of ENT section"(Chinese Medical Association,people’s Medical Publishing House)to develop.The one who met the following conditions:1 complained of hoarseness;2 signs:endoscopy showed unilateral or bilateral vocal cord pedicled or sessile polypoid growths,could be put in the research.A total of 60 patients were included in the study,stroboscopic laryngoscope,voice range profile,voice analysis and voice handicap index check scale were perfected at the time of the first diagnosis.All the patients had no other basic diseases.All microscopic laryngoscope operations were completed by the same otolaryngology doctor and the anesthesia physician under the same equipments.All patients in the postoperative 1 week and 4 weeks were followed up once again back to the hospital,perfecting the stroboscopic laryngoscope,voice range profile,acoustic analysis and voice handicap index check scale to gain statistics compared to the statistics gained at the first diagnosis.All patients avoid using continuous and high intensity sound during follow-up periods.All preoperative and postoperative examinations were performed by the same physician.All statistics are completed using SPSS 19.0 software.Result:1.VHI:VHI results were significantly better than the preoperative level,the difference was statistically significant(P<0.05).Because the vocal cords which had been operated lweek later was still slightly swollen and the patient’s had mental stress,so 4 weeks after the surgery the VHI improvement was most obvious,the difference is statistically significant(P<0.05).2.Voice acoustic analysis:(1)fundamental frequency perturbation(Jitter):The difference was statistically significant(P<0.05)before and after 1 week and 4 weeks after surgery,the difference was statistically significant(P<0.05)at 4 weeks after surgery.(2)amplitude perturbation(Shimmer):1 weeks and 4 weeks after operation Shimmer value was smaller than preoperative,the difference was statistically significant(P<0.05).4 weeks after the surgery the improvement of Shimmer values was better than 1 week after surgery,and the difference was statistically significant(P<0.05).(3)Normalized noise energy(noise energy normalized,NNE).The difference was statistically significant(P<0.05)before and after 1 week and 4 weeks after surgery,the difference was statistically significant(P<0.05)at 4 week after NNE.(4)NHR(ratio Harmonice-noise,NHR):At 1 weeks and 4 weeks after operation,NHR was improved in different degree,4 weeks after the surgery the improvement of NNE was better than 1 weeks after surgery,and the difference was statistically significant(P<0.05).3.VRP:(1)in terms of frequency of vocal cord polyp patients before the maximum frequency,the minimum frequency,frequency range 1 weeks after operation there were no significant difference(P>0.05);the average postoperative 4 weeks in patients with the highest frequency than before the surgery increased,the minimum frequency lower than the preoperative,frequency range than before surgery significantly enlarged,there were significant differences(P<0.05).(2)strength,vocal cord polyp patients before and 1 weeks after operation at the maximum sound intensity and sound intensity,the minimum intensity range had no significant difference(P>0.05);and after 4weeks with average maximum and minimum value of sound intensity is lower than before the surgery,and the average sound intensity range expanded,the difference was statistical significance(P<0.05).Conclusion;The difference between before and after surgery in patients with VHI,voice acoustic analysis and VRP related parameters through the study,we think that the voice acoustic analysis and VHI VRP,with the voice of quantitative evaluation function,can effectively evaluate the curative effect of vocal cord polyp surgery,is an important means of voice function detection. |
