ObjectiveDynamic laryngoscope and DIVAS2.5 voice analysis device were used to detect the vocal index of vocal polyps,vocal cords leukoplakia and larynx cancer patients,to explore the diagnostic and differential value of voice acoustic parameters in benign and malignant vocal cord diseases,and to find a more sensitive evaluation index.By using the dynamic laryngoscope voice analysis device,we examined the voice recovery of 40 male patients with unilateral vocal cord polyps after one months undergoing operation.Methods1.The subjects included 30 cases of unilateral vocal cord polyps,25 cases of vocal cords leukoplakia and 25 cases of glottic carcinoma between 2016.9-2017.12.Among them,30 cases of normal voice were used as the control group.The laryngeal dynamic voice analysis software produced by Xion Limited was used to detect 3 times of the same vowel /a:/ for each subject before operation,and the average value was obtained.The detection indexes include fundamental frequency perturbation(Jitter),amplitude perturbation(Shimmer),normalized noise energy(NNE),longest pronunciation time(MPT),and dysphonia severity index(DSI).Two independent sample t tests were carried out on the collected data with SPSS19.0 software,and P < 0.05 was statistically significant.2.A unilateral vocal cord polypectomy was performed in the experimental group under the dynamic laryngoscope.After the operation,avoid of using sound and attention to voice health care for 1 weeks.The voice acoustic test was performed 1 month after surgery,and the above parameters were obtained.SPSS19.0 software was used to measure two independent samples t test on the data,Statistical significance was considered as P <0.05.Results1.Compared with normal subjects,the Jitter,Shimmer and NNE values of glottic laryngeal carcinoma group,vocal cord leukoplakia group and vocal cord polyp group,while MPT and DSI values decreased significantly(P<0.05),with statistical significance.Compared with vocal cord polyps and leukoplakia patients,glottic laryngeal carcinomaincreased with Jitter,Shimmer and NNE values,while MPT and DSI values decreased.The difference was statistically significant(P<0.05).The Jitter and shimmer values of the patients with vocal cord leukoplakia and vocal cord polyp increased,and the DSI value decreased.The difference was significant(P<0.05),which was statistically significant,while MPT and NNE had no significant difference(P>0.05),and there was no statistical significance.2.In the vocal cord polyp group,the values of Jitter,shimmer and NNE decreased,and MPT and DSI values increased significantly compared with the data before operation.The difference was statistically significant(P<0.05).There was no significant difference in Jitter,shimmer,NNE and MPT between one months after operation and normal subjects(P>0.05),but no statistical significance.DSI was still lower than the normal subjects,the difference was statistically significant(P<0.05).It shows that one months after the vocal cord polyp operation,the voice quality was obviously improved,but still failed to recover completely.Conclusion1.Acoustic analysis of dynamic laryngoscope is a noninvasive examination,it can not only be used as a screening tool for patients with hoarseness,it can also be properties of vocal fold lesions for diagnosis and differential.And Jitter,Shimmer and DSI are more sensitive indicators.The possibility of malignancy should be considered when the value of Jitter is 4 times higher than normal and Shimmer is 2 times higher than normal.2.The above voice parameters can be used as an objective basis for the evaluation of voice function before and after the treatment of vocal cord hyperplasia.The vocal quality of the vocal cord polyp was obviously improved one months after the operation,but it still failed to recover completely.
