Objective:Observeing changes in the indicators of N-acetylaspartate(NAA)/creatine(Cr),and choline-containing compounds(Cho)/Cr and ml Ns/Cr,and the(δ+θ)/(α+β)values of q EEG of patients with diffuse axonal injury who therapyed by two different pressure of hyperbaric oxygen,to study the effect of different pressure hyperbaric oxygen(HBO)therapy on clinical outcomes and prognosis of patients with diffuse axonal injury.Methods:Fifty cases of diffuse axonal injury admitted into our hospital from October2014 to October 2015 were randomly divided into 3 groups: the control group(10case),the lower hyperbaric oxygen group or 0.20 MPa group(20case)and the higher hyperbaric oxygen group or 0.25 MPa group(20case).The control group was just given routine treatment,the lower HBO group received 0.20 MPa HBO therapy in addition to routine treatment after vital signs became stable,and the higher HBO group received0.25 MPa HBO therapy in addition to routine treatment.Neural functions of the 3groups were evaluated by using GOS and NIHSS.Magnetic resonance spectroscopy was used to observe changes in indicators of NAA / Cr,Cho/Cr and m INs/Cr;Data(including α、β、δ、θ and(δ+θ)/(α+β)of q EEG were collected both before and after treatment.Then,changes in the botained data were compared.Results: Statistical differences could be seen in the scores of GOS and NIHSS,when comparisons were made between the groups(p<0.05 or p<0.01).As compared with those of the lower HBO group and the control group,the levels of NAA/Cr,Cho/Cr,m INs/Cr in the splenium of corpus callosumin,and NAA/Cr and Cho/Cr in basal ganglia for the 0.25 MPa HBO group were either elevated or decreased,with statistical significane(p<0.05 or p<0.01);The(δ+θ)/(α+β)values of q EEG both reduced among three groups after 6 weeks and the degree of the change : 0.25 MPa group> 0.20 MPa group> the control group.the difference was statistically significant(p<0.05);Moreover,there was a significant correlation between the(δ+θ/α+β)values of q EEG with the GOS of DAI.Conclusions:Compared with the lower HBO,early higher hyperbaric oxygen treatment could obviously enhance metabolism of brain tissues in the splenium of corpus callosumin and basal ganglia and accelerate the slow wave of q EEG to reduce or increase fast wave of q EEG,obviously improve the neurological function,to improve the prognosis.
