Objective: To study the findings of diffuse axonal injury (DAI) on radiology and evaluate their value. Methods: A retrospective study was conducted on 46 patients with DAI on the imaging and the clinic features, the relationship between location and clinical prognosis ,GCS were evaluated. Results :Totally 91 lesions were found in 46 cases including 68 lesions in cerebral white matter ,4 lesions in basal ganglia ,10 lesions in corpus callosum ,2 lesions in cerebellum ,7 lesions in brainstem. The lesions involved the medial part of brain in 17 cases.The lesions did not involve the medial part of brain in 29 cases. 12 cases had been cured ,31 cases straightened up ,2 cases showed a state of plant , 1 cases died. There were 8 cases got 13~15 GCS points,12 cases got 9~12 GCS points,and 26 cases got lower than 8 points. Conclusion :The lesions involving the medial part of brain had worse prognosis and got lower GCS than lesions not involving the medial part of brain. Analyzing comprehensively the images and the clinical features in diffuse axonal injury ,MRI is the most valuable examining method for DAI ,to a certain extent ,can provide evidence for early diagnosis ,therapy , monitoring and prognosis.
