The Study Of Varicocele Afterperation With TCM Constitution Research

ObjectiveBased on the investigation of varicocele of spermatic vein ligation for preoperative and postoperative TCM Constitution,help doctors to guide patients with varicocele after surgery in the pathogenesis and targeted to biased constitution regulation,and provide clinical basis for the theoretical basis for improving the varicocele patients with hand from the constitution of TCM after rehabilitation and improve semen quality.MethodsThrough the questionnaire survey of TCM Constitution of patients with varicocele before and after surgery,according to TCM constitution establishment scale,the scale by Yin deficiency,Qi deficiency,qi stagnation,gentleness,phlegm,Yang deficiency,blood stasis,the intrinsic quality of 9 small table.Comparing the above information,analysis of the changes of the physical operation of spermatic vein ligation.The general clinical data collection:collected from hospitalized patients with surgical treatment of varicocele in each branch and Zhuhai branch of the Department of Urology Guangdong Province Traditional Chinese Medical Hospital andrology general information and scrotal ultrasound,semen routine data etc..The above data were analyzed and compared to obtain the distribution of postoperative physical fitness of patients with varicocele.Finally,the data are collected through SPSS22.0 statistics and analysis.Results1.This study included 92 cases of hospitalized patients,of which 18-30 years a total of 52 people(56.6%);36 were 31-40 years old(39.1%),4 were 41-50 years old(4.4%);64 cases of patients with bilateral after operation(69.6%),28 cases of patients with unilateral after operation(30.4%),in which all patients with unilateral surgery parts are left,according to the statistics of surgical site a total of 156 cases of spermatic vein ligation.There were 92 patients with left lateral surgery,including 12 patients with I degree(13%),53 patients with II degree(57.6%),27 patients with III degree(29.3%).There were 64 patients on the right side,including 29 patients with I degree(45.3%),33 patients with II degree(51.6%),and 2 patients with III degree(3.1%).There were 26 cases(28.3%)with high ligation of spermatic vein under laparoscope.There were 66 cases(71.7%)with ligation of spermatic vein under microscope.The BMI index is divided into 4 groups,respectively is<18.5 group(underweight):13(15.2%),18.5-23.9 group(normal):51(54.4%),24-27.9 group(fat):27(29.4%),more than 28 groups(obese):1(1.1%).2.Spermatic vein ligation patients before the physical distribution ratio from big to small is damp heat in 26 cases(28.3%),20(21.7%)patients with Qi deficiency and blood stasis in 16 cases(17.4%),10(10.9%)cases of Yang deficiency and qi stagnation in 9 cases(9.8%),and 4 cases(4.4%flat),3(3.3%)cases of yin deficiency,phlegm dampness in 2 cases(2.2%),the intrinsic quality of 2 cases(2.2%).Among them,the dampness heat,Qi deficiency,blood stasis and yang deficiency are the most widely distributed before the ligation of spermatic veins.The physical distribution proportion of patients 3 months after ligation of spermatic vein 3 from big to small was 25 cases of damp heat(27.2%),21(22.8%)patients with Qi deficiency,blood stasis and 17 cases(18.5%),11(12%)cases of Yang deficiency and qi stagnation in 8 cases(8.7%),3 cases of yin deficiency(3.3%),and 3 cases(3.3%),quality of phlegm dampness in 2 cases(2.2%),the intrinsic quality of 2 cases(2.2%).Among them,the dampness heat,Qi deficiency,blood stasis and yang deficiency are the most widely distributed in the 3 months after the ligation of spermatic vein.4.After the ligation of spermatic vein,there was no significant difference between the TCM Constitution and the preoperative physical fitness(P>0.05).5.The volume of the left testis after the ligation of the spermatic vein was correlated with the postoperative fitness(P<0.05).The left testis volume less than 15ml in patients with blood stasis(36.4%),Qi deficiency(27.3%)and Yang(27.3%)higher proportion of left testicular volume was 15ml in patients with damp heat(34.3%)higher proportion.From a single physical form of testicular size ratio,the higher the damp heat in the left testicular volume was 15ml the proportion of patients;the left testicular volume<15ml and more than 15ml in the proportion of patients with Qi deficiency and blood stasis;Yang deficiency,left testis volume less than 15ml ratio significantly higher than 15ml patients.6.After the ligation of spermatic vein,the improvement of VAS score was related to the postoperative physical fitness(P<0.05),among which the improvement of VAS score in the patients with blood stasis was the worst.7.Ligation of spermatic vein can improve the quality of semen in patients with varicocele.8.After spermatic vein ligation,the improvement of semen was related to the postoperative physical fitness(P<0.05),the improvement of semen concentration in the patients with Qi deficiency was the worst,and the sperm motility in the patients with blood stasis was the worst.Conclusion1.Spermatic vein ligation before and after operation in patients with TCM Constitution distribution of damp heat,Qi deficiency,blood stasis and yang deficiency accounted for the highest proportion of spermatic vein ligation in patients with TCM Constitution in a certain period of time will not change.2.Spermatic vein ligation on the left testicular volume less than 15ml in patients with blood stasis,Qi deficiency and yang deficiency higher proportion of left testicular volume was 15ml in patients with a higher proportion of damp heat.3.After the ligation of spermatic vein,the testicular pain in the patients with blood stasis syndrome is the worst.4.After spermatic vein ligation,the spermatic fluid concentration of the patients with Qi deficiency is the worst,and the sperm vitality of the patients with blood stasis is the worst.
