| OBJECTIVE:In assisted reproductive technology(ART),the study of how to improve the clinical pregnancy rate and reduce the number of transplanted embryos is mainly aimed at female factors,but the role of sperm in embryonic development is equally important.How to choose better sperm and how to choose more potential for the development of embryos for transplantation has become the focus of attention.With the application of intracytoplasmic morphologically selected sperm injection(IMSI)technology,the researchers often observed at least one vacuole in the sperm nucleus and found that these vacuoles in the sperm head IMSI has a poor prognosis.The aim of this study was to investigate the effect of the number and size of vacuoles in the sperm head on embryonic development in IMSI using time-lapse.MATERIALS AND METHODS:A total of 243 MⅡ oocytes from 21 infertile patients were selected for inclusion in the reproductive department of Dalian Municipal Woman and Children Medical Center from January to December 2015,and IMSI was injected The(N = 79)injected spermatozoa containing 1-2 small vacuoles(d<1um),group A(n=48)injected spermatozoa without vacuoles,group B(n = 79)),Group C(n = 75)injected spermatozoa containing more than two small vacuoles(d<1um),group D(n =41)injected spermatozoa containing more than one large vacuole(d>lum).The general situation of patients in the group,such as male and female age,infertility,infertility factors,total Gn and the number of Gn days,semen routine parameters;the four groups were the general analysis of the laboratory,such as the average The number of eggs,the fertilization rate,the cleavage rate and the D3 excellent embryo rate were analyzed.The kinetics parameters of the four embryos were analyzed by time-lapse.Application of SPSS 17.0 system software for data analysis,measurement data application of variance test,count data application test to P<0.05 for the difference was statistically significant.RESUIT:The average age of the patients was 33.8 ± 2.1 years old,the mean age of the men was 35.6 ± 4.2 years old,the average infertility period(2.0±1.4)years was the average age of the patients.Among them,14 cases were primary infertility,7 cases were secondary infertility,8 cases were male only,11 cases were male factor combined with tubal factors,average Gn(2645.4 ± 216.3)IU,average Gn days(11.5±1.2)days The The total parameters of semen samples were semen volume(3.4±0.6)ml,sperm density(6.1 ±1.2)106/ml,sperm motility(26.9±2.6)%;②The four groups of laboratory general conditions,such as 2PN fertilization rate,(P>0.05).However,the odds ratio of D3 in group D was significantly lower than that in group A(66.66%vs 23.53%,P>0.001).③ The A and B,C and D(T4,h)in group A was significantly higher than that in group D(40.14 ± 4.33 vs 51.16 ±11.16,P = 0.012),and there was no significant difference between the two groups(P>0.05)(T8,h)was significantly earlier than that in group D(46.61± 3.43 vs 59.74 ±10.34,P = 0.009).The time(t8,h)of group A reached 8 cell stage was significantly earlier than that of group D(P<0.05).(2)The cell stage(CC2,h)in group A was significantly shorter than that in group D(9,91±2.23 vs 16.33±6.78,P = 0.043)(0.33 ± 0.06 vs 1.99 ± 0.06,P = 0.024).There was no significant difference in the time of the three groups of embryos from 3 cells to 5 cells(CC3).There was no statistically significant difference in time from 5 cells to 8 cells(S3).CONCLUSION:The effect of sperm head vacuoles on embryonic early development can not be ruled out.Injection of vacuoles without vacuoles reaches 4 cells significantly shorter than injections of large vacuoles,and sperm chromosomes or DNA damage containing large vacuoles may have gone beyond embryos The ability to repair the size of the sperm head of the spermatozoa before the IMSI injection has a significant effect on the embryonic development of early embryos. |
