Background and ObjectiveObstructive sleep apnea syndrome is a very prevalent syndrome in clinic.The possible causes of this disease include genetic factors,anatomical factors,neurohumoral factors,obesity and many other factors.OSAS can cause serious complications in cardiovascular and cerebrovascular,endocrine and other systems,so that the quality of life of the patients has declined,and even life-threatening.OSAS is now considered as an independent risk factor of a variety of systemic diseases.It has been proved that OSAS can promote tumor formation and progression.And there is a certain relationship between OSAS and head and neck tumors.Laryngeal and hypopharyngeal cancers are common malignant tumors in head and neck tumors and also in the Department of Otolaryngology.It is reported that the majority of patients with laryngeal and hypopharyngeal malignant tumors also have various symptoms such as sleep disorders,daytime sleepiness and so on.Patients with OSAS and head and neck cancers at the same time may have higher incidence of postoperative complications.And the expression of tumor-associated genes in patients after treatment was decreased.This shows that the effective treatment of OSAS have a certain significance on the prevention and treatment of cancer.Therefore,whether these patients with OSAS should be diagnosed and treated as soon as possible,thereby reducing its role in the occurrence and development of tumors and reducing the complications of tumor treatment.The aim of this study is to investigate the correlation between OSAS and laryngeal and hypopharyngeal malignancies,and to provide some guiding significance for clinical diagnosis and treatment.Methods50 patients with laryngeal and hypopharyngeal cancer between February and September in 2016 in the department of otolarynology head and neck surgery of the first affiliated of Zhengzhou university were collected for the study.All patients underwent portable monitor device.The data include sleep Apnea Hypopnea index(AHI),mean oxygen saturation and the lowest oxygen saturation.OSAS was defined as an AHI value≥5 events per hour.Compare the OSAS and non-OSAS patient groups in the respects of age,sex,BMI,tumor location and tumor stage.Logistic regression equation was established by using OSAS as dependent variable,age,sex,BMI,tumor location and tumor staging as independent variables to analyze the risk factors of OSAS in laryngeal and hypopharyngeal malignant tumors.ResultsAccording to the diagnostic criterion of OSAS,43 of 50 patients were diagnosed as OSAS in our study,and the incidence of OSAS is about 86%,the mean score of AHI was(15.81±11.87)events per hour,the mean average oxygen saturation was(92.52±5.10)%,the mean lowest oxygen saturation was(72.80±10.78)%.OSAS and non-OSAS patient groups were similar with respect to sex,Body Mass Index,tumor location and tumor grade(P> 0.05).Logistic regression analysis shows that age is the independent risk factor of OSAS in patients with laryngeal and hypopharyngeal cancer.ConclusionThere is a high prevalence of OSAS among patients with laryngeal and hypopharyngeal cancer,and age is a risk factor.And there is no significant relationship between OSAS and BMI,tumor location and tumor grade in the patients we studied.
