Traumatic joint contracture is a common clinical problem that seriously interfere with the patient’s motor function,activities of daily living and quality of life.In recent years,static progressive stretch has been widely used to treat traumatic joint contracture.Clinically,stretching torque depend on patient’s subjective feeling,stretch time is usually 20 to 40 minutes,the objective experimental basis is inadequate.Research purpose:To find the best time for the static progressive stretch treatment of traumatic knee joint contracture in rats,so as to provide theoretical basis for clinical treatment.Research method:Twenty-eight male Wistar rats with a body weigh 200 g were chosen.1% pentobarbital sodium(100g/0.5ml)were applied in intraperitoneal anesthesia and partly got hair removal with NaS.After cutting the muscles by scalpel,Expose the knee joint and part of the femur and tibia,a 4 square millimeter piece of articular cartilage was removed in the femoral articular surface with 2 mm wide osteotome,and punctured knee posterior articular capsule,caused the local damaged bleeding,The kirschner wire was used through the femur and tibia,make crus fixed in the biggest flexion,30 days later the kirschner wire was removed.The 28 Wistar rat model that had been made traumatic joint contracture were randomly divided into 4 groups with 7 in each group.Group A was the control group which would remain unstretched;group B,C and D would be stretched for 20 minutes,30 minutes and 40 minutes correspondingly.All the three stretched groups were used the same torque of 0.04 Nm,stretching angle of 90°,stretching value of 0.8N.The stretching cycle was 30 days and each stretching was done every other day and stretching was done for 15 times in all.The range of motion(ROM)was measured for all the group one day after the creation of the model,the day after stretching for 5 times,10 times and 15 times.Therefore,the ROM in the right knee joint was measured in all for 4 times.All the measuring was done by the same person who was not related to this experiment.All the four groups were given a gait analysis(Catwalk)on the 2nd and 30 th day respectively after the model was created.Gait analysis was carried out for 3 times in each rat,the average value was taken.All data in this experiment are analyzed by the statistical software SPSS22.0 and are tested by Shapiro-Wilk method.Then the data those are distributed normally are tested by t test,and those are not distributed normally are tested by nonparametric.The measurement data are all shown by “mean number ± standard deviation”.The comparison before and after the static progressive stretch are tested by the paired samples t test.The comparison between groups is conducted by two independent sample t test.Tests were statistically significant with p<0.05.Results:There was no statistical significance(p > 0.05)about ROM between the four groups before treatment.After 30 days’ treatment,theROM in four groups had a very big improvement,with significant difference compared with before treatment(P < 0.001),the ROM in C and D groups improved significantly better than group A(p < 0.05),the improvement in group C was superior to the B and D groups(p < 0.05).At the early 20 days in four groups,the ROM were quickly improve,but at later 10 days the improvement was slow.The Catwalk data indicated that after the stretching the stride of front claws,swing speed,maximum contact area and average pressure all increased than those before the stretching.The best effect was gotten in group C and the difference was statistically significant(p<0.05).Conclusions:(1)Joint contracture can be formed after fixing the traumatized knee joint for one month.(2)The moderate static progressive stretch has a significant treatment effect on traumatic joint contracture.(3)The effect of static progressive stretch for 30 min is superior to that for 20 min and 40 min.(4)The moderate static progressive stretch improve gait of rats with traumatic joint contracture.
