The Effect Observation Of Electroacupuncture Combined With Rehabilitation For Treatment Of Shoulder Subluxation After Stroke

Objective:To observe the clinical effect of electroacupuncture combined with rehabilitation for treatment of shoulder subluxation after stroke,so as to find a simple,safe and effective method for the treatment of shoulder subluxation after stroke in clinic.Methods:60 patients according to the inclusion criteria with shoulder subluxation after stroke were randomly divided into extrameridian points group and routine acupuncture group and rehabilitation group,20 cases in each group.Three groups of patients with the same basic treatment,extrameridian points group and routine acupuncture group were treated with electroacupuncture stimulation combined with rehabilitation therapy,while extrameridian points group were selected the supraspinatus and the deltoid muscle on the acupoint,routine acupuncture group selected the acupuncture points on the deltoid muscle rehabilitation group was treated with pure rehabilitation therapy,and the three groups were treated for 4 weeks.Before and after treatment,were compared three groups of patients’ shoulder pain score(VAS),the minimum distance between the acromion and the humeral head(AHI)and the changes of upper limb motorfunction(FMA),and then comprehensive statistical analysis.Results:1 After treatment,the shoulder pain in the three groups patients have different degrees of relief,there was significant difference before and after treatment(P<0.01),the degree of shoulder pain relief extrameridian points group compared with the rehabilitation group,the difference was obviously significant(P<0.01).Shoulder pain relief degree extrameridian points group was better than the routine acupuncture group,the routine acupuncture group was better than rehabilitation group,there were significant differences(P<0.05).2 After treatment,the distance of shoulder subluxation in the three groups decreased in different degrees,the difference was statistically significant before and after treatment(P<0.01),in which the extrameridian points group obvious reduced than the rehabilitation group,the difference was significant(P<0.01).While the extrameridian points group shoulder subluxation reduce was compared with routine acupuncture group,the routine acupuncture group and rehabilitation group comparison,there were significant differences(P<0.05).3 After treatment,the three groups of patients’ upper limb motor function scores were significantly increased,in which the extrameridian points group was obvious improved than rehabilitation group,there was significant difference(P<0.01).While extrameridian points group was improved than the routine acupuncture group obvious,the routine acupuncture group was improved than the rehabilitation group obvious too,there were significant differences(P<0.05).4 The three groups of patients’ comprehensive efficacy by Ridit analysis after treatment,Rextrameridian points group<Rroutine acupuncture group<R rehabilitation group,that is,the comprehensive effect of the extrameridian points group is better than the routine acupuncture group,and the routine acupuncture group is better than the rehabilitation group.Conclusion:1 Electroacupuncture combined with rehabilitation therapy and rehabilitation therapy alone can improve the symptom of shoulder subluxation after stroke,but electroacupuncture combined with rehabilitation therapy is better than simple rehabilitation therapy.2 Electroacupuncture stimulation of the acupoints on supraspinatus muscle and deltoid muscle,are largely alleviated the ipsilateral shoulder pain,reduce shoulder joint subluxation,promote the upper limb motor function recovery,the therapeutic effect is better than stimulation of the acupoints on the deltoid muscle alone.
