| Objective:The purpose of this study is to analyze the CT characteristic of benign and malignant thyroid nodules and explore the value of CT in the differential diagnosis of benign and malignant thyroid nodules. Methods:The CT findings including margin, capsule, calcification, cystic change, the degree of enhancement, lymph node enlargement, the gender, age and A/T of 150 cases were retrospectively analyzed and in comparison with the data of the pathology, the features of benign or malignant nodules were compared. The risk factors of malignant thyroid nodules were analyzed by Logistic regression analysis. Results:Significant statistical differences were existed between benign or malignant nodules in the aspects of the age,A/T,the number of nodules, margin, encapsulation, cystic change and lymph node enlargement(p<0.05).For the diagnosis of malignant nodule, the sensitivity of non﹣capsulation andill ﹣ defined margin were 96.9% 、 76.9% respectively, the specificity were62.4% 、 51.8% respectively, the positive and negative predictive vanes were66.3%、54.9% and 96.4%、74.6% respectively. The differences of CT values of thyroid nodules with different characteristics were statistically significant in plain scan, arterial phase and venous phase(p<0.05).Multivariate logistic regression analysis indicated that no capsule, lymph node enlargement and A/T were the independent risk factors for the malignant thyroid nodules(p<0.05).Conclusion: CT scan is an important method for differential diagnosis of benign and malignant thyroid nodules. This study demonstrated the feasibility of CT in the differential diagnosis of benign and malignant thyroid nodules. There were differences in the age,A/T,number of nodules, margin, capsule, cystic change and lymph node enlargement of thyroid nodules with different characteristics. No capsule, ill﹣defined margin,no cystic change, lymph node enlargement and solitary nodule indicate the possibility of malignancy. |
