Objective: Due to the lack of early screening of ovarian cancer were hidden,late once found in most patients,the ovarian cancer with high incidence,high mortality,low survival rate,etc.So looking for strong specificity,high sensitivity,simple,convenient,non-invasive and repeatable detection method is the focus of the scholars both at home and abroad at present.This study was to explore the reliable way of diagnosing the malignant Ovarian Cancer and clinical Ovarian Benign Tumor.Methods:Take a total of 248 inpatients including 49 ovarian cancer cases and 199 ovarian benign tumor cases,and 40 cases of normal women as the research objects,who were admitted to Yijishan Hospital of Wan Nan Medical College during a period from.in the time of October 2015-January 2017.The objects were divided into group A(ovarian malignant tumor group)and group B(ovarian benign disease group),group C(healthy control group).Collected the research object of the age,menopausal status,color doppler ultrasound results(including tumor volume,maximum diameter,border,blood supply,pouch or solid,and presence of pelvic cavity accumulates fluid),Carbohydrate antigen199,Carbohydrate antigen125,Human epididy-mis secretory protein 4,past medical history,previous ovarian surgery,Past Marital and Fertile History,postoperative pathological diagnosis,grading and staging of tumor,and calculated Risk of Ovarian Malignancy Algorithm.Through the analysis of the single index or Multiple Indexes value of ovarian cancer diagnosis,todiscuss the clinical value of indicators,and provide resources for the clinical research in the future.Conclusion: 1.In this study,49 cases of ovarian malignant tumor group;199 cases of ovarian benign tumor group;healthy control group of 40 cases,a total of 288 cases.The age of ovarian cancer patients was 16-81 years old,the average age was53.94 ± 11.33 years old;the median age was 53 years old.The age of the ovarian benign disease group was 18-75 years,with an average age of 40.88 ± 15.36 years.The median age was 41 years.The healthy control group was 20-78 years old with an average age of 37.82 ± 12.28 years and a median age of 37 years.2.Serum CA125,HE4 levels,ROMA index in the ovarian benign and malignant tumors and healthy control group the difference between the three groups was statistically significant(P= 0.000 <0.001).But,there was no significant difference in CA199 between the three groups(P = 0.468> 0.05).The levels of serum CA125,HE4 and ROMA in ovarian cancer group were significantly higher than those in ovarian benign tumor group(P= 0.000,0.000,0.000 <0.001).The levels of serum CA125,HE4 and ROMA in the ovarian malignant tumor group were significantly different from those in the healthy control group(P= 0.000,0.000,0.000 <0.001).There was no significant difference in serum CA125,HE4,level and ROMA between the ovarian benign tumor group and the female healthy control group(P= 0.086,0.725,0.886>0.05).3.CA125,HE4,ROMA index of the ROC curve,calculate the area under the curve(AUC),the results is 0.857,0.823,0.846,(P=0.000,0.000,0.000<0.001),But the AUC area of CA199 of only 0.467(P = 0.468> 0.05).This shows that serum CA125,HE4,ROMA index alone in the early diagnosis of ovarian cancer,the accuracy is high.There is a certain clinical significance.But,We don’t recommend the use serum CA199 alone as an early diagnosis of ovarian cancer indicators.4.The sensitivity and negative predictive value of color Doppler ultrasonography combined with ROMA were 93.88% and 96.99%,respectively,which indicated that the clinical value of early diagnosis of ovarian cancer was higher.Result: 1.CA199,CA125,HE4,ROMA values,ultrasound A single application,ROMA sensitivity and negative predictions have obvious advantages,HE4 specificity,positive predictive value,negative predictive value,accuracy advantage is obvious.2.Multi-index joint detection : Ultrasound + ROMA combined with CA199 + CA125,CA125 + HE4,ultrasound + CA125 + HE4 ideal,can be used as the ideal combination of ovarian cancer screening.3.The ROC curves of serum CA125,HE4 and ROMA were drawn,calculate the area under the curve(AUC),the results were 0.857,0.823,0.846,(P=0.000,0.000,0.000<0.001).ROMA diagnosis of benign and malignant ovarian cancer has a high clinical value.
