Object:To explore the clinical characteristics and prognostic factors of primary gastric lymphoma(PGL).Method:Methods a total of 42 patients with primary gastric lymphoma admitted to the Second Affiliated Hospital of Nanchang University from January 2011 to December2016 were retrospectively analyzed.The collection of clinical and biochemical indicators recorded in the Excel table,for the application of SPSS22.0 statistical software to analyze the collected data through the results,count rate or porpertion;the survival rate was calculated using the Kaplan-Meier method,single factor survival analysis using Log-rank test,COX analysis of multi factors on the single factor analysis P < 0.05,finally found its independent factors affecting the prognosis.Result:Results Of the 42 patients,the most frequent symptom were abdominal pain,abdominal distention,nausea and weight loss.The most commonly involved site were the corpus,and the autrum.Endoscopic biopsy confirmation rate was 27%.The predominant pathologic subtype in our study was diffuse large B cell lymphoma(DLBCL),followed by mucosaassociated Lymphoid tissue(MALT)lymphoma.Chemotherapy is the main modality to treat PGL.The 1,3and cumulative 5year Survival rates were 82% 76%and 69%.In univariate survival analysis,level of LDH,Ann Arbor staging,age,international prognostic index(IPI),and B symptom were obviously correlated with the prognosis all(P <0.05).Ann Arbor staging and international prognostic index(IPI)were independent prognostic factors in multivariable analysis(P < 0.05).Conclusion:Primary gastric lymphoma(PGL)is a kind of relatively rare cancer and easily misdiagnosed due to its unspecific signs in digestive tract,occurs in older men,maleand female incidence ratio of 1.1:1.Abdominal pain,abdominal distension is the most frequent symptom,followed by regurgitation belching,vomiting,abdominal mass,fever,and weight loss.B symptoms are rare.Bleeding,perforation is the complication.Electronic endoscopy is the main diagnostic method at present,but separate endoscopic biopsy positive rate is low,easily misdiagnosed as gastric cancer or ulcerative lesion;lesion is the most easy to involve the gastric corpus and antrum,the ulcerative lesions are the most common lesions.Single lesions are more common than Multiple lesions.DLBCL and gastric MALT lymphoma is the main pathological type.In recent years,the majority of patients can be diagnosed in early staging,high degree of malignant lymphoma with low malignancy more easily in late diagnosis.Chemotherapy is the main modality to treat PGL.There are many factors that affect the prognosis of PGL,the conclusions tumor staging and IPI score were independent prognostic factors for survival in patients with PGL.
