Correlation Research Of Peripheral Blood Neutrophils/Lymphocytes Ratio In Endometrial Adenocarcinoma

Objective:This study aims to count and analyse the blood routine of endometrial adenocarcinoma,endometrial hyperplasia,pelvic inflammatory disease and healthy women,to ascertain the value of neutrophils/lymphocytes ratio(NLR)used to diagnose endometrial adenocarcinoma,and to explore the cut off value of neutrophils/lymphocytes ratio in the diagnosis of endometrial adenocarcinoma.Methods:This study used retrospective analysis methods.Select 124 cases of endometrial adenocarcinoma,141 cases of endometrial hyperplasia,71 cases of pelvic inflammatory disease which were diagnosed in the department of gynecology at The First Affiliated Hospital of Kunming Medical University,and 137 cases of healthy women during the period of January 2010 to December 2016.Record the fasting blood routine of research object before treatment.Applied statistics to analyse and compare neutrophils/lymphocytes ratio of endometrial adenocarcinoma group,endometrial hyperplasia group,inflammatory disease group and normal control group.Obtain the cut off value of neutrophils/lymphocytes ratio in the diagnosis of endometrial adenocarcinoma by using ROC curve.Results:1、The neutrophils/lymphocytes of endometrial adenocarcinoma group is 2.39±1.08,the neutrophils/lymphocytes of endometrial hyperplasia group is 2.02±1.18,the neutrophils/lymphocytes of inflammatory disease group is 5.77±7.49,the neutrophils/lymphocytes of normal control group is 1.74±0.66.The differences of comparing any two groups are statistically significant,inflammatory disease group>endometrial adenocarcinoma group>endometrial hyperplasia group>normal control group.2、In the group of endometrial hyperplasia,the neutrophils/lymphocytes of endometrium without atypical hyperplasia group is 2.05±1.38,the neutrophils/lymphocytes of atypical hyperplasia of endometrium group is 1.99±1.04.There is no statistically significant difference comparing the two groups.3、In the group of endometrial adenocarcinoma,area under the curve of ROC(AUC)is 0.631,P=0.000,which has statistically significant difference,and when the cut off value of NLR is 1.690,the sensitivity and specificity are 100%and 33%.In the group of endometrial hyperplasia,AUC is 0.406<0.5,there is no practical significance.In the group of inflammatory disease,AUC is 0.899,P=0.000,which has statistically significant difference,and when the cut off value of NLR is 3.675,the sensitivity and specificity are 77.5%and 92.0%.The value range of NLR used to diagnose endometrial adenocarcinoma is 1.690-3.675.Conclusions:1、NLR has diagnostic value in endometrial adenocarcinoma.Its specificity is low,but the sensitivity is extremely high.Combining other auxiliary examinations may help for the early diagnosis of endometrial adenocarcinoma.2、NLR is of no value in discriminating endometrium without atypical hyperplasia and atypical hyperplasia of endometrium.3、NLR is expected to be as one of the auxiliary means of diagnosing endometrial adenocarcinoma,which is needed more in-depth research to confirm.
