| Objective: To understand the present situation of iron deficiency,analyze the relevant factors to family and diet of children diagnosed with IDA in children aged6 ~ 12 years old in Lanzhou and Dongxiang.Control study was made by Han and Dong Xiang nationality which lived in Lan Zhou and Dong Xiang to know the situation in different regions and nationality.Methods: The children aged 6~12 years old were selected by stratified random sampling from four primary school in Lanzhou and Dongxiang during October in2015.The children were selected to carry out physical development indicators,venous blood collection and questionnaire.Through the analysis of hemoglobin,mean corpuscular volume,mean corpuscular hemoglobin concentration,mean hemoglobin,serum iron,serum ferritin and other indicators,34 cases of children with IDA were selected.34 ordinary children were selected have the same age,sex,nation and region as control group.At last univariate analysis and multivariate Logistic regression analysis were used to investigate the family factors and dietary behaviors between two group.Control study was made which lived in Lan Zhou and Dong Xiang.Results:1.Current status of iron deficiency in aged 6~12 years old children in Lanzhou and Dongxiang The prevalence rates of iron redution,IDA and ID in 396 children were 12.63%,8.59%,21.21%.The prevalence rate of iron reduction,IDA,ID in Dongxiang was higher than that in Lanzhou,the difference was statistically significant(P﹤0.05).Dongxiang nationality prevalence rate of iron reduction,IDA,ID was higher than that of Han nationality,the difference was statistically significant(P ﹤ 0.05).The prevalence rate of IDA was lower in senior children than that of junior(P﹤0.05),and the decrease of iron was not related to age(P﹥0.05).There was no significant difference in prevalence rate of iron reduction,IDA and ID between male and female(P﹥0.05).2.Single factor analysis and comparison between IDA group and non IDA group Family factors The overall distribution for monthly income,parents,education and the knowledge of IDA have the different location between two group.(monthly income Z=-3.887,P ﹤ 0.05;parents,education Z=-4.790,P ﹤ 0.05;the knowledge of IDA Z=-4.077,P﹤0.05).Family eating habits The overall distribution of family dietary structure between the IDA group and non IDA group have the different location(Z=-2.527,P﹤0.05).There is a statistically significant difference in the wok cooking food and drinking habits.(wok cooking food c2=2.946,P﹤0.05;drinking habits c2=2.435,P﹤0.05).Dietary behavior The overall distribution for infant feeding,picky eaters,meat eating and snacking of IDA have the difference location between two group.(infant feeding Z=-2.873,P﹤0.05;picky eaters Z=-2.078,P﹤0.05;meat eating Z=-2.992,P﹤0.05;snacking Z=-1.970,P﹤0.05).Motion factor The overall distribution of movement time have the same location between two proup.(movement time Z=-1.995,P﹤0.05).3.Comparing the factors of IDA in Lanzhou and Dongxiang The overall distribution for monthly income,parents,education,dietary structure and infant feeding have the different location between two region.(monthly income Z=-2.373,P﹤0.05;parents,education Z=-2.905,P﹤0.05;dietary structure Z=-2.222,P﹤0.05;infant feeding Z=-2.689,P﹤0.05).4.Logistic regression analysis of family and diet factors relevant to IDA The prevalence rate of IDA was significantly affected by infant feeding(OR=8.945,95%CI:1.097~9.485)(P=0.007),the time of eat snack every week(OR=2.688,95%CI:1.052~6.866)(P=0.039)and picky eaters(OR=2.770,95%CI:1.664~11.553)(P=0.017).Conclusion: The prevalence rate of ID was better than before especially the children in Dongxiang,but the hidden iron deficiency is still the main problem of nutritional iron deficiency.We should increase the income of the national economy and strengthen the propaganda of prevention and health care.The key to preventiron IDA in school-age children is carry out health education and change the bad diet behavior and dietary structure. |
