A chronic non-communicable disease(referred to as chronic diseases)has become an important issue and the burden of disease also has been the hot topic of the health system in the world.The effect of chronic diseases on the social and economic aspects increased gradually,due to the increased prevalence year by year,people's healthy lifestyle,personal health literacy,living environment and other factors.The comprehensive prevention and control management of chronic diseases based on the government-led,professional organization support,the whole society participating,therefore,the effective chronic disease prevention and control system for long-term chronic disease management model was built.With the implementation of comprehensive prevention and control of chronic disease management system,chronic disease prevention and control network and improving health resources in primary health care institutions and it has been effective use efficiency of health services are important for adopt to increasing demand of chronic disease management.In this paper,both the status of comprehensive prevention and treatment of chronic diseases were summarized,the evaluation index system of chronic disease management were constructed,the effect of comprehensive prevention and control of chronic diseases were analyzed,and then countermeasures and suggestions were put forward to realize the long-term management of chronic disease prevention and control.In this paper,we set up the index systems of comprehensive management of chronic diseases,which is scientific and sustainable using the Delphi method and "structure-process-result" evaluation model according to systematic,practical,workable principle.From the analysis of indicator system,we suggest that the system consists of three aspects: structure,process and result.There are thirty-two indicators,including three indicators on first level,six indicators in the second level and twenty-three indicators in the third level.Based on indicators of content and data from2010 to 2016 related indicators obtained by survey,the effect of DanTu integrated management of chronic diseases were assessed.Results of the comprehensive evaluation indicate that the implementation effect from 2013 to 2016 has generally exceeded the standard value based on the indicator.We found that the input level and the overall situation of the team are good,and the region "3 + x" health team was achieved;the contractual health management was strengthened.In the process of implementation,the implementation of hypertension and chronic disease management are in good condition,and the health management has a high degree of implementation of health education and health records.However,we found that the allocation of resources in the number of public doctors,the number of beds and chronic disease prevention and control funding needs to be further strengthened,the use of health records in health management remains to be strengthened.The overall output effect of the frequent participation in the number of physical exercise is still less.The health effects of high blood pressure and diabetes could have upside.In this study,we established the index system of integrated management of chronic diseases and analyzed the effect of long-term development in Dantu District,and put forward the countermeasures and suggestions to improve the comprehensive management of chronic diseases.The main views include improving the comprehensive prevention and control of chronic disease management level,promoting universal participation in health management,optimizing the allocation of resources management of chronic disease,improving the long-term management quality of comprehensive prevention,and strengthening the management of chronic diseases management.
