Objective 1.To observe the effect of home blood pressure monitoring on blood pressure management in hypertensive patients;2.To explore the application of different forms of home blood pressure monitoring in blood pressure variability, blood pressure level and loss rate management in hypertensive patients;3.The analysis of different forms of home blood pressure monitoring outside the hospital management of hypertension patients in different ages. Methods In this study,342 cases of hypertensive patients treated in our hospital from June 2016 to December 2016 were selected as the research object,The patients were divided into home blood pressure monitoring group and control group according to the voluntary principle;Home blood pressure monitoring group further divided into remote monitoring group and traditional group based on the principles of voluntary again;Respectively, with different outside hospital management methods for follow-up management of each group of patients, blood pressure variability, blood pressure and loss of follow-up between groups were compared after 6 weeks;at the same time, both two group patients were divided into age < 55 years old group,55 years old age group, the aged 55 to 65 years old group and age >65 years old group, blood pressure variability, blood pressure and loss of follow-up between the patients with different ages were compared. Results 1.After followed up for 6 weeks, there was a significant difference in the level of blood lipid and the attendance rate between home blood pressure monitoring group patients and control group patients, the weight,BMI,systolicblood pressure and diastolic blood pressure of the two groups were significantly lower than those of the baseline;2.After 6 weeks of follow-up,the standard deviation of systolic blood pressure, systolic blood pressure coefficient, mean systolic blood pressure and mean diastolic blood pressure were significantly lower in remote monitoring group than traditional group(6.69±1.81 VS 8.30±3.13,P=0.016;0.05±0.02 VS 0.06±0.02,P=0.048;127.66±6.16 VS 129.51±9.09,P<0.001;75.99±6.37 VS 76.58±8.25,P<0.001);The decrease of systolic blood pressure, diastolic blood pressure, standard deviation of diastolic blood pressure, systolic blood pressure coefficient, mean value of systolic blood pressure, diastolic blood pressure standard deviation, diastolic blood pressure coefficient, mean diastolic blood pressure, uric acid in remote monitoring group was significantly higher than that of traditional group.3.In the traditional group, patient lost was46.1%,but in the remote monitoring group it was only 10.0%.4.After 6 weeks of follow-up,the symptoms of systolic blood pressure variability and loss of follow-up were significantly different of patients indifferent ages in traditional group;but in the remote monitoring group, there were no statistically significant differences in blood pressure variability and loss of followability between patients of different ages. Conclusions Home blood pressure monitoring can can effectively observe the level of blood pressure in patients with hypertension and reduce the rate of re-treatment;and remote monitoring of home blood pressure monitoring in patients with different age of hypertension in their blood pressure variability, blood pressure and loss rate was significantly better than traditional home blood pressure monitoring.
