OBJECTIVES To investigate the current conditions of home morning blood pressure monitoring(HBPM)in patients with hypertension from second and third grade hospitals in Chongqing.METHODS The survey was conducted through paper questionnaire in patients with essential hypertension.Patients came from second and third grade hospitals in Chongqing during February2015 and March2016.Standard questionnaires were adopted to collected data and office blood pressure(BP)was measured by training qualified clinicians.Survey process was controlled strictly.Data was analyzed through SPSS 19.0 software.RESULTS In hypertensive patients included(n=2056),1.Men accounted for 47.7%.There were no significant differences in the ratio of HBPM(68.7%vs65.8%)and in the ratio of patients achieving standard BP level among HBPM patients(34.6%vs34.2%)(P > 0.05)between male and female patients.2.Younger than 65 years patients,≧65 years patients accounted for 41.5%,58.5%,respectively.There were no significant differences in the ratio of HBPM(67.2%vs67.1%)and in the ratio of patients achieving standard BP level among HBPM patients(31.9%vs36.2%)(P > 0.05)between <65 years patients,≧65 years patients.3.Patients with secondary education degree or lower accounted for 90.6%,and patients with junior college and above education degree accounted for 9.4%.There was no significant difference in the ratio of HBPM(66.8%vs71.0%)(P>0.05)between them.The ratio of achieving standard BP level in patients with junior college and above education degree among HBPM patients was higher than in patients with secondary education degree or lower(46.0%vs33.1%)(P<0.05).4.Patients familiar with hypertension diagnostic criteria accounted for 31.2%.The ratio of HBPM(71.0%vs65.4%)and the ratio of achieving standard BP level(39.9%vs31.7%)was higher in Patients familiar with hypertension diagnostic criteria than in patients not familiar(P<0.05).5.Patients familiar with that hypertension causes complications accounted for 58.9%.There was no significant difference in HBPM rates between patients familiar with that hypertension causes complications and patients not(67.4%vs66.9%)(P>0.05).The ratio of achieving standard BP level was higher in Patients familiar with that hypertension causes complications than patients not among HBPM patients(41.2%vs24.6%)(P<0.05).6.Patients taking single-agent,dual drugs and triple or more drugs to control BP accounted for 48.9%,33.4% and 17.7%,respectively.The ratio of HBPM among HBPM patients(68.6%vs66.4%vs64.8%)and the ratio of achieving standard BP level(34.5%vs32.0%vs38.6%)were no significant differences among these patients(P>0.05).7.Patients from the main city third grade class A hospitals and from the county second grade class A hospitals accounted for 37.4%,62.6%,respectively.The ratio of HBPM was higher in the main city third grade class A hospitals than in the county second grade class A hospitals(75.0%vs62.5%)(P<0.05).The ratio of achieving standard BP level among HBPM patients was higher in the main city third grade class A hospitals than the county second grade class A hospitals(52.9%vs21.1%)(P<0.05).8.There were no differences in the ratios of achieving standard office BP level between HBPM patients and not HBPM patients(38.2%vs36.0%)(P>0.05).The ratio of achieving standard office BP level was higher in the patients achieving standard morning BP level than in the patients not achieving(62.5%vs25.4%)(P<0.05).CONCLUSIONS The rates of HBPM are not sufficient in second、third grade hospitals in Chongqing.Patients familiar with hypertension criteria,and from main city tertiary hospitals may be more likely to monitor morning BP at home.Patients with high culture degree,familiar with hypertension criteria or that hypertension causes complications,and from main city tertiary hospitals have higher rates of achieving standard home morning BP level,while the rates of achieving standard office BP level were higher in patients achieving standard home morning BP level.
