| Low back pain(LBP)is one of musculoskeletal disease,with the lifetime prevalence of as high as 84%.Clinically,pain and dysfunction are two common indicators that characterize lumbar function characteristic in patients with LBP,and usually use a scale method to evaluate these indicators.However,pain and dysfunction as a subjective feeling,not only there are large individual differences,but also is very easy to be influenced by external environment.Therefore,the method that use scale to evaluate pain and dysfunction has some shortcomings.Presently,there is a lack of an objective and quantitative method to evaluate lumbar function characteristic in patients with LBP in clinical practice.However,surface electromyography(SEMG)signals can evaluate muscle objectively and quantitatively.Therefore,in order to look for an objective and quantitative method to evaluate lumbar function characteristic in patients with LBP,at the same time,to provide theoretical basis for the research and development of wearable health detection system of the spine,and provide auxiliary reference basis for the diagnosis of LBP.Multiple features of SEMG signals on lumbar muscles of sixty patients with LBP were extracted in this paper,EMG features and clinical evaluation indicators of lumbar function were quantified subtly.The quantitative research of lumbar function characteristic was implemented and it has the important value of clinical application.First of all,the selection criteria of LBP patients was described and the clinical evaluation scale of lumbar function characteristic was described in detail.It can provide theoretical basis for the clinical evaluation of lumbar function characteristic in patients with LBP in the following papers.Secondly,the pretreatment method of SEMG signal was studied.Based on the generation mechanism of SEMG signal,the common mathematical model was analyzed,the composition of SEMG signal acquisition system was introduced,the acquisition process of SEMG signal on lumbar muscles was described.In our experiment,SEMG signal of lumbar muscles of thirty healthy people and sixty patients with LBP were collected,then SEMG signal was pretreated by Chebyshev type I filter.Thirdly,the feature extraction methods of SEMG signal were studied.In this paper,the feature extraction algorithm of time domain,frequency domain and time-frequency domain were introduced.Combining with the SEMG signal that were collected in theexperiment,the features of SEMG signals were extracted,such as mean square root,integral electromyography,median power frequency,median frequency,wavelet coefficient a4 maximum value and wavelet coefficients a4 energy.Finally,lumbar function characteristic of LBP patients were studied and analyzed based on the features of SEMG signals.First of all,the source and general information of this experiment objects were described,and the lumbar function in patients with LBP features were assessed by clinical scales in this experiment.Then,these features of SEMG signals of lumbar muscles in LBP patients were studied by statistical method and the clinical scale assessment results of lumbar function characteristic were quantified subtly.A linear regression model between pain grade and features of SEMG signals in LBP patients was established.The correlation between the dysfunction level and the features of SEMG signals in LBP patients was investigated.At the same time,the threshold of SEMG signal features of LBP patients was analyzed based on ROC curve.The assessment of lumbar function characteristic in patients with LBP based on SEMG signal features were studied in this paper systematically and quantitatively.Results showed that the SEMG signal can be used as objective method to assess the lumbar function characteristics in patients with LBP quantitatively,the root mean square,integral electrical power value,median frequency,median frequency,maximum wavelet coefficient a4 and wavelet coefficient a4 energy can be used as effective index for the assessment of lumbar function characteristic. |
