| ObjectiveThe aim of this paper has determined to analyse annual health examination and unhealthy behavior factors of international seafarers to know about the epidemic trend and epidemic features of abnormal BMI(overweight and obesity),high blood pressure and abnormal blood glucose(impaired fasting glucose and hyperglycemia)and its risk factors among international seafarers,So that we can offer some strategies and measures for prevention and cure of international seafarers and raise concerns of related department for special crowd disease control and prevention.Material and methodsThe current study was consist of two methods,one is a retrospective analysis the other is a Cross-sectional study.Retrospective analysis is a population-based study using such annual health check data from international seafarers working with Guangzhou shipping company.Between 2008 and 2015,a total of 42,149 annual health examinations were collected to analysis the epidemic trend of abnormal BMI/blood pressure/ blood glucose.The overall prevalence rates were age-standardized using state-level age and male data from the 2010 national census data.Changes in abnormal BMI,high blood pressure and abnormal blood glucose between the surveys by age were analysed using Chi-square test.χ2 test.A cross-sectional was conducted at a Sentinel hospital.Survey covered the anthropometry,Blood biochemical test and questionnaire to record the basic information in detail,including Anthropometric data(Height,weight,blood pressure),fasting blood glucose data,sociodemographic factors,occupational factors,living habits,the medical histories and family histories of disease,Awareness of chronic disease.Univariate and multivariate logistic regression analysis was performed to assess the risk factors for abnormal BMI,high blood pressure and abnormal blood glucose.Result1.There were 4699,4963,4881,5071,5190,6073,6165,health examination records from 2008 to 2015 After age standardized adjusted,the prevalence of overweight was 31.28%,32.99%,31.33%,32.27%,34.97%,36.02%,36.13% and 36.51%,respectively;the prevalence of obesity was 3.48%,3.83%,4.49%,6.61%,6.86%,8.02%,7.68% and 9.63%,respectively;the prevalence of high blood pressure was 26.68%,28.26%,24.77%,18.72%,12.89%,8.81%,8.61% and 6.17%,respectively;the prevalence of IFG was 7.70%,11.25%,14.76%,13.60% and 8.55%,respectively;the prevalence of hyperglycemia was 2.10%,2.53%,3.09%,2.80% and 2.28%,respectively.2.797 international seafarers were carry out the questionnaire survey,smoking rate was 43.04%,drinking rate was 68.88%,frequently drinking rate was 9.41%,Occasional drinking rate was 40.40%,little drinking rate was 50.19%;94% with a regular eating,19.82% tasting salty,77.54% and 88.44% of them have pork and vegetables every day,insufficient intake of fruits,eggs and products,milk and dairy products accounted for 37.64%,64.37% and 71.14%;frequently exercise on aboard was 18.07%,occasionally exercise was 57.09%,rarely or never exercise was 24.84%,the main sports on board were walking,running and playing table tennis;41.41% of seafarers have 2 hours-4 hours rest time on board a day;the passing rate of knowledge of chronic diseases was 35.88%.3.The unadjusted prevalence of abnormal BMI,glucose and blood pressure was 42.3%,17.94% and 11.54% respectively.The overall age standardized prevalence of which was 43.68%,20.06% and 12.75% respectively.Marital status(P=0.022,OR=1.62),Job duration at sea(P=0.013,OR=1.17)was risk factors for abnormal BMI.Monthly income(P=0.023,OR=0.34)was a protective factor for abnormal BMI.Job duration at sea(P=0.009,OR=1.44)was risk factors for high blood pressure.Job duration at sea(P<0.001,OR=1.56)and drinking(P=0.005,OR=2.24)are risk factors for abnormal blood pressure.Conclusion 1.The rate of overweight and obesity with a trend of rising year by year from 2008-2015.Abnormal BMI was related to Job duration at sea,marital status and monthly income.2.Abnormal blood pressure with a trend of declining year by year from 2008-2015.Abnormal blood pressure was related to Job duration at sea.3.The rate of impaired fasting blood glucose,hypertension increased firstly and then decreased from 2011-2015.Abnormal blood sugar was related to Job duration at sea and drinking frequency.4.Chronic disease-related risk factors,such as smoking rate,drinking rate,physical inactivity and insufficient intake rate of fruits,eggs and products,milk and dairy products remain in a high level. |
