Epidemiologic Characteristics And Intervention Study Of Unintentional Burns In Rural School-aged Children In Northern Guizhou Province

Objective To explore the characteristics and influencing factors of unintentional burns among school-aged children,and analyze the knowledge level and influencing factors of burns among school-aged children and their parents in rural Northern Guizhou Province,so as to provide a basis evidence for the prevention strategies.Methods A stratified sampling method was used to recruit 1842 rural children from 9 schools(6 primary schools and 3 middle schools),self-reported burns during the past 12 months and relevant concerns and influencing factors of burns were collected by questionnaires,the epidemiological characteristics of burns were analyzed,and Logistic regression analysis was used to find out the influencing factors.At the same time,the knowledge level of burns was investigated,and the influencing factors were analyzed by multiple linear regression analysis.One county were randomly selected from the three counties which were surveyed at the baseline stage,and then one of the primary schools and one of the secondary schools were selected.Children and their primary guardians were sampled in the county as the survey objects.The KAP level of children's burns and their influencing factors were analyzed.After the baseline survey,students from one county were randomly selected from the three previous baseline-survey counties;after intervention by SFI(school-family-individual)model,the effect of intervention was evaluated.Besides questionnaires,students(40 middle school students and 40 pupils,respectively)were interviewed and group discussed.Results(1)The burn incidence of all interviewed children was 12.7%(95% confidence interval [95% CI] =11.20%-14.21%).We found that burn incidence had a trend to increase with the increasing grade,while a trend to decrease with increasing knowledge scores on burns.The top two causes of burns were hot liquids(36.3%)and hot object(29.5%).(2)The main risk factors for burns included 3-4 grade(OR=1.84,[95% CI]=1.20-2.80),5 grade(OR=2.00,[95% CI]=1.33-2.99);female(OR=1.70,[95% CI]=1.29-2.25);left-behind kids by parents who were working in cities(OR=1.35,[95% CI]=1.02-1.80);and poor mother education(OR=2.43,[95% CI]=1.61-3.69).(3)More than 30% children had little knowledge about how to provide first-aid and preventive measures after burns;the total awareness rate of knowledge burns relatively was 82.65%,but nearly one-third of students failed to reach 80% accuracy rate.After a group interview,we found some children have insufficient knowledge about burns prevention and first aid;grade,single child in a family,non-left behind,and maternal education were influencing factors for burns-related knowledge(the absolute values of normalized regression coefficients ? were 0.206,0.054,0.048 and 0.047,respectively),the student grade has the greatest influence on the knowledge score of the burn,and the influence of the mother's education degree was relatively small.(4)The awareness rate of parents to pediatric unintentional burns' KAP(Knowledge,Attitude,Practice)was only 61.82%,which the main risk factor was the principal guardian of education level(?=0.126).Secondly,the student grade distribution(?=0.099)and the family of left-behind children(?=0.088).(5)After intervention by the SFI model,the incidence of burns in the intervention group decreased from 8.82%(41/465)to 4.49%(21/468)before intervention(P=0.008),the total awareness rate of burns-related knowledge increased from 79.07% to 82.74%(P<0.001).(6)The classification tree model analysis showed that the main influencing factor of unintentional burns was gender of children,the most significant factor affecting boys' burns was the level of their mothers' education level,and the main factors influencing girls' burns are their family economic situation.Conclusion The incidence of pediatric unintentional burns was high in rural southwest China;Grade,gender,left-behind children and mother's education degree were the main influencing factors of burns;And the level of burns knowledge of children and their parents need to improve,and schools,families,local public health agencies should put efforts into health education on burn prevention,particular intervene and improve knowledge level of injury prevention for the left-behind children and these families of low-mother's education degree,to improve children's burns prevention knowledge comprehensively,it should be beneficial to create a safe environment to prevent children far away from burns.
