Objective:The clinical features of ischemic J waves were found in the electrocardiogram of patients with acute coronary syndrome,the diagnostic value and clinical significance of the adverse cardiovascular events in patients with acute coronary syndrome were discussed.Methods: Selected 149 cases of acute coronary syndrome patients in January 2015-August 2016 in our hospital and the first hospital in Lanzhou University,these patients after admission electrocardiogram and dynamic electrocardiogram,according to electrocardiogram and dynamic electrocardiogram with or without ischemic J wave division of ischemic J wave group and non-ischemic J wave group.Discussion: 1.the relationship between ischemic J wave and non-ischemic J wave group,such as age,sex,24-hour mean heart rate,etc;2.Which the incidence of ischemic J waves in the three types of acute coronary syndromes is highest;3.Comparison of the common site of ischemic J wave in electrocardiogram of patients with acute coronary syndrome;4.4.Is the occurrence of ischemic J wave is prone to ventricular arrhythmia;5.The relationship between ischemic J wave and ventricular arrhythmia and coronary heart.Result: There were 63 cases of ischemic J wave in 149 patients,the mean heart rate was(75.5 ± 16.7)/ min,the non-ischemic J wave was 86 cases,the mean heart rate was(66.0 ± 12.1);The detection rate of ischemic J wave was(52.9%,27.6%,28.6%,P <0.05)in the three types of acute coronary syndrome(ACS)in three types(STEMI,NSTEMI,UA);The incidence of ischemic J wave in the lower wall was 61.8%,the incidence in the anterior wall was 29.5%,P <0.05;The incidence of ischemic J wave and ventricular arrhythmia was 41.3%,which was much higher than that of nonischemic J wave combined with ventricular arrhythmia(19.8%,P = 0.04);Ischemic J wave combined with ventricular arrhythmia and the incidence of left coronary artery lesions as high as 69.2%.Conclusion: In this study,ischemic J wave is more likely to occur in acute ST-elevation myocardial infarction The mean heart rate of ischemic J wave group was faster than that of non-ischemic J wave group.ST-elevation myocardial infarction in the most easily found in electrocardiogram(ECG)ischemic J wave and under the ECG lead wall appear the most easily[1].Due to the limited number of cases collected,left coronary artery,circumflex artery,right coronary artery to separate the number of cases less,that is combined into the left and right coronary artery.ACS with ischemic J wave in patients with increased risk of ventricular arrhythmias and occurred in the left coronary artery in a higher proportion,indicating that the emergence of ischemic J waves and patients with ECG instability is closely related to ischemia The ion mechanism of J-wave is the increase of potassium ion to epicardial movement during myocardial ischemia,leading to the formation of potential difference between epicardial and outer membrane,which may increase the possibility of occurrence of cardiac malignant events...
