Objective: To study the correlation between detrusor function state and bladder spasm after operation in patients with benign prostatic hyperplasia, which can be a certain extent in the preoperative to predict and assess the occurrence of postoperative bladder spasm and its severity, reduce the blindness of clinical treatment, and to provide theoretical basis for postoperative prevention, make the patients benefit from operation to the greatest extent. Methods: 103 patients with benign prostatic hyperplasia(BPH)who underwent transurethral plasmakinetic resection prostate(TUPKRP)were enrolled from November 2015 to December 2015 in the department of Urology, affiliated hospital of Qinghai University, and were confirmed by histopathology as benign prostatic hyperplasia. On the basis of preoperative urodynamics existence of bladder detrusor instability(DI)was divided into A,B two groups.Group A(experimental group)for bladder detrusor instability group(DI group),group B(control group)for bladder detrusor stability group(DS group).Finish and record general situation of the two groups, like age, international prostate symptom score(IPSS),maximum urinary flow rate(Qmax),catheter retention time, operation time and bladder rinse time, etc. The number, incidence and severity of bladder spasm during the postoperative catheterization of the two groups were observed, and statistical analysis of the data by statistical methods. Results: The incidence of postoperative bladder spasm in the DI group was 77.78%,the incidence of DS group was 53.06%,there was a statistically significant dissimilarity between the two groups(P <0.05),the incidence of postoperative bladder spasm was higher in the DI group. In the number of postoperative bladder spasm, the DI group was significantly higher than the DS group, there was a statistically significant difference between the two groups(P<0.05).For the severity of postoperative bladder spasm pain score,The DS group had a lower score, contrasted with the DI group, the discrepancy was obvious(P <0.05).Conclusions: The preoperative detrusor function state of BPH patients was associated with the occurrence of bladder spasm after TUPKRP.Patients with DI group were more likely to develop bladder spasms after surgery, not only with higher rates and more severe. So the detrusor function state can be grasped through the preoperative urodynamic examination in clinical practice, and need to pay special attention to those patients with detrusor instability, emphasis on postoperative prevention of intervention to prevent or reduce the occurrence of postoperative bladder spasm. |
