| Objectives To explore the design of puerperium women self-management network intervention model based on application,which maternal as the center,combined the puerperium self-management with transitional care,so that puerperium women can control their own health,breastfeeding,neonatal care and other knowledge and skills.Let the puerperium women adapt to the changed role,improve their quality of life.Methods70 puerperium women were assigned into two groups.In experimental group,patients received routine care plus the management guided by APP’s nursing management model;In control group,patients were given routine care only.Then women’ depression、quality of life(QOL)and breastfeeding self-efficacy of the two groups were evaluated after 6 weeks’ intervention.SPSS 17.0 was used by data analysis,the descriptive statistics,chi-square and T test statistic methods were used.Results According to the test,there’s no statistical difference between the experimental group and control group between social population informatics variable and baseline outcome(P>0.05),indicating these two groups were comparable.After the intervention,the depression’s scores of two groups have no difference(P > 0.05),the experimental group’s scores of QOL and breastfeeding self-efficacy are better than the control group’s(P<0.05).Conclusion Transitional care model based on APP can improve the knowledge of self-management and neonatal care,improve the confidence.Establish close relationship among doctors、nurses and puerperal.Improve self-management behavior and quality of life.So that the quality of hospital services to further enhance.Therefore,Transitional care model based on APP can be viewed as an effective management model. |
